Everybody/everyone - Minasan
(I) am (name) - (Watashi) wa (Namae) desu.
(My) nickname is (nickname) - (Watashi no) adana wa (adana) desu.
(I) am from (place) - (Watashi) wa (tokoro) kara desu.
Example:
Hajimemashite.
Watashi wa (Surname, name) desu.
Baguio City kara kimashita.
Dozo yoroshiku.
You can also follow the example if your introducing yourself.
Note: DESU means is, am, are which indicate a state of being. Desu is likewise a sentences ender. (Pronounce it like "DES" if your from/in Tokyo and "DE-SU" if your from/in province in Japan.
