Nice name! Tho, it sounds pretty Japanese-ish and it's fine if she's Japanese, but if she's Malaysian you should go to a more Malaysian-ish name.

Species: Human
Mhm.
Age:First appearance and the original series -- 11 years old
Galaxy series -- 14 years old
Seems pretty reasonable.
Appearance: Original Series -- A petite girl short, yet long at the front, autumn brown hair with caring sepia eyes.
In the Galaxy series, Tsukiya grew more lithe than she was petite and her hair grew longer, like waist length for the back section while the front section grew only in the middle of her abdomen. Her eyes stayed the same, except more bored looking than it was caring.
Her appearance seems fine to me.
Personality: She is an understandable and caring person. She sometimes radiates with a mother's aura but she is also a bit naive. She enjoys torturing herself and or make others suffer, in a way, she has a masochistic and sadistic sides to it.
In the Galaxy, Tsukiya has this snarky and stern attitude, more open than when in the original series.
Okay, if she's a caring and understanding person why does she enjoy making people suffer? And I don't quite understand the 'enjoys torturing herself' part too.
Family: An only child. Her parents are deceased thus she was put in an orphanage where BoBoiBoy used to live with his parents.
The whole dead parents thing are kinda overdone tbh, but I don't really care about that. I'm curious on how her parents died.
Powers: Doesn't have a power but to make it up, Tsukiya has this Japanese fisher knife that was actually a red blazing sword with black steel trimmings around the handle to the tip of opposite blade.
The sword can absorb powers, change its appearance and strike its opponents back with the power absorbed. The sword also keeps the powers but it will break if the amount of powers becomes too unbearable. Alas, the sword can regenerate itself in some time. It can also produce sounds.
It is actually a gift and a momentum from Tsukiya's parent to her before they left her in an orphanage when she was a year old.
The sword is kind of overpowered, tho, the whole breaking and mending back together is fine. But why does it change its appearance? It'll make sense if it's alien technology... But is the changing appearance necessary? It'll also make sense if it's a really important sword that got stolen so she had to change its appearance every once in a while.
Fears: Automatonophobia, fear of humanoid robots.
Claustrophobia, fear of being enclosed in a small space or room and unable to escape.
Okay then.
Weaknesses: I haven't thought of it but it's the same with fears
How about when she doesn't have her sword with her? Make that one of her weaknesses if she relies on her sword when it comes to fighting.
Likes: Books (Going book hunting with BBB to be specific), Desserts (cakes especially), Animal (reptiles the most), Skating (both rollers and ice), Letters, Music and hanging around BoBoiBoy.
Okay then.
Dislikes: Dishonest people, Perverts, Sour food (or anything that has sourness in general), cramp spaces, humanoid robots and History
Mhm, seems reasonable.

Little Lieutenant by Admiral Tarung, Sai and Shielda
That nickname from Boboiboy is really cute! I can imagine her singing Boboiboy a lullaby every night and that's why he calls her Lullaby, aww.
Relationship status:
BoBoiBoy -- Lover and childhood best friend
Gopal -- Brotherly Sisterly relationship
Yaya and Ying -- First female friends (her age), Rivals ( in exams) and best friends.
Fang -- Rivals and have a deep brotherly and sisterly relationship.
The whole OC x Canon shipping is fine by me, and her relationships with the other characters seems fine also!

I have to say, Tsukiya has potential! I'd love to see a fanfiction of her sometime and she seems to fit in the BBB universe just fine. Tho, I recommend fixing her personality a bit since it clashes a bit. But other than that, she's a good oc!
