Sans is a MONSTER and not a HUMAN, so he doesn't follow HUMAN biology like a HUMAN skeleton. Same with Gaster and Papyrus.

"Thanks for the tea, darling~" Mettaton chirped, as he was sitting at a small little table in a cafe, with Sans and his husband Papyrus. Of course, the person that was serving them was none other than Frisk, still in that all too familiar maid's outfit, that she had to wear as part of her work. The adult woman gave a wide smile in return, while asking if there was anything else she could get them. "Of course not, darling. This is more than enough."
"THANKS FOR SERVING US TODAY, FRISK!" Papyrus cheered, while Frisk giggled, pointing out how it was part of her job... True, there were extra people that had been hired to work at the cafe... So, it's not like it was always just her and Kai anymore that served people. But, to tell the truth, the two new people who had been hired to work here, were some of the bitchiest adults Frisk had ever met. It made her wonder what her bosses were thinking. But talking behind people's backs didn't really stop them from being unemployed, she guessed.
"welp, i better head back." The other skeleton spoke up, as he was standing up out of his seat, with his coat pulled back over his shoulder. He gave a small peck to Frisk on the lips as he smiled. "promised chara and asriel that i would get back to play a game of cards with em. and ya know how she gets when i'm late." He laughed, while Frisk giggled alongside him, as she really did know Chara all too well. Despite the passing of time, that girl really had never changed. To be honest, no one really had. In a negative sense, anyway. Everything had only been changing for the better.
Sans and Frisk were still very much in love. There wasn't a day in their lives together that either of them felt like they regretted getting married. True, they had had some bumps in the road here and there, even when they did finally get married, but that was the way with every relationship that you cared about. Sometimes you had to fight and battle for the things you cared for to make them work. They both knew that they didn't regret a single ounce of blood, sweat and tears that went into what was their current lives. "Oh, you lovebirds." Mettaton sang teasingly, as the husband and wife were staring at each other quite lovingly. Sans rolled his eyes.
"like ya are one to talk." He shot back, seeing as Mettaton and Papyrus' seats were sat quite close together. It was lucky for them that Sans had managed to bite back his protective nature of his younger brother, because if past him had seen them sitting as close as they were now, he'd probably be teleporting between them to move them over a little. With another chuckle, he glanced back to Frisk. "i'll see ya back at home, alright?"
"Okay! Don't get too worked up over that card game, though." She smiled, giving him another quick peck before he went to leave...
Yes, as you probably expected, Sans was still very much a part of the Mafia. True, ever since getting married and having his kids grow up, he probably hadn't had as much of an involvement as he had before his and Frisk's relationship even existed... And even now, sometimes it just felt like he went to work to have a good time, more than to do the things that they were actually employed for. To tell the truth, his dad probably wanted to give him a bit of a break - give more tasks to others so he no longer had to work as hard. They had still got new members of the years after all... Any time a new person joined, it made him think of the first time he ran into Frisk, and she became part of the Mafia.
Sometimes it was easy to forget that she had no home before that day. Nowhere to go. That her parents had been taken from her, and she had been all on her own. How drastically different their lives might have been, he wondered, if they had never met. But... Tibia honest with you, (yes, that was in fact a pun), he didn't really like to think about such thoughts. Because he knew had him and Frisk never met, he probably wouldn't even still be here right now, and some of the best moments of his life had come from being able to fall in love with her.
Like having kids. Something he thought he'd never have in a million years. But here he was, thinking about how nice it would be to see his teenage children when he got home later... It was insane to think that so much time had already passed since they had been born... They were both 17 years of age... That really did make the skeleton feel old. 38 years of age, huh? Man... How time had flown. That meant that Frisk was now 37, which made him feel even older.
19 and 20 they had both been, the first time they had come face to face. And now here they both were, still together, nearing their 40's as a married couple. Sans never knew he could be so lucky.
Despite having fun being stuck in his thoughts of sentiment and nostalgia, Sans knew he couldn't be late. If he was, he'd likely never hear the end of it from the human girl. So of course once he arrived at Grillby's bar, he gave the flame monster behind the bar a small wave and a smile, before stepping into the familiar elevator, which would allow him to go further down, and to the base below.
"Ah, you actually made it on time!" Chara waved from the table that she was sitting at, alongside with Asriel. It was always the quietest area of the headquarters, the place where some people would just come down to relax, unless you wanted to alternative, which was Grillby's bar, (which they did hang out in from time to time. Sans rolled his eyes with a small grin, as he went to sit down. "Almost thought you were going to be late. It's a long drive from where you live."
"well, that's why i left early." Sans pointed out with a chuckle. It was probably one of the many other reasons why Gaster didn't expect Sans to work as much as he used to... He lived much further away, still living in his and Frisk's dream house where they had originally gone to stay safe from Undyne and the rest of her crew. They hadn't moved house since, because they really did still love living there. And it could be a hassle, driving back and forth all the time. Not to mention, Sans did have a family that he needed to spend time with, and to protect. Sans still recalled the conversation he had with his father awhile back, when it came to what happened on his and Frisk's honeymoon.

"I'm glad that you and Frisk are safe. After I heard what had happened..." Gaster murmured, as he was sitting behind his office desk. Sans was sitting in the chair opposite him, making himself comfortable after his dad had offered him a seat. "It's hard to believe James would try something like that. He must have been that desperate to kill you." He pointed out, while Sans shrugged.
"wasn't anythin' i couldn't handle." He said. Though, he had been shaken up, thinking that Frisk could have almost been killed in that whole situation. Andrew's brother was a fucking dirtbag, and it blew his mind how two family members could be so insanely different. Sans had made the horrible mistake of not trusting Andrew for so long, thinking that he was probably a bomb waiting to go off. Little had they known, that the person they really should have looked out for was his twin, James.
"I know. You survived after all." Gaster weakly smiled. "But I think from now on, you really should avoid getting into trouble as much. For the sake of Frisk; your family. After all, even if he jumped off that ship, there's no guarantee that he even died." The older skeleton man pointed out. "So be cautious."

But here they were, many years later, and nothing had happened since that day. So, either James was taking his sweet, sweet time to come up with a new plan, or he was for certain: dead. Either way, Sans didn't care. He knew he would be prepared for that piece of trash on any day of the week if he decided to show his fucking face again.
"alright, let's start this game." Sans grinned widely, once the cards had been dealt between the trio. Chara smirked in return.
"Get ready to lose!"
