The nurses found Peeta months later, his physical state decreasing rapidly. The nurses began to worry, and question if he was going to survive.
His heart was faltering, signing that he didn't have much time left.

The chemicals the capital had used on him during torture had finally taken their toll.

They told Peeta to start saying his goodbyes, for they didn't think he had that much longer. There weren't many people he actually cared about, so the goodbyes where pretty easy, for him anyway.

He lived his final moments to the fullest. His breaths grew shallower each and every day, and he smiled on it, knowing his girl on fire was waiting for him

And so one week later, in the quite of dawn Peeta passed away peacefully with a smile on his face, his spirit joining hers in the echoes of eternity

As he took his last breath he saw Katniss standing before him, reaching out her hand, she took him by the hand and led him away to a place they could live in peace forever.

Everyone finally understood the reasons for Peeta's visions of Katniss. It wasn't just his imagination. It was a sign that he was near the end of his journey. And Katniss, his beloved girl on fire, had come to take him home.
