This refers to Perseus 'Percy' Jackson. The eldest god in question is Poseidon. He is one of the oldest gods and part of the big three (Hades, Poseidon, and Zeus).

Percy turns eighteen against all odds (the numerous near-death experiences) thrown at him.

And see the world in endless sleep

Percy saw the world, referring to Manhattan, was put to sleep by Morpheus. This allowed the war to take place without any mortal interference.

The hero's soul, cursed blade, shall reap

The cursed blade was Annabeth's knife, which was given to her by Luke prior to them making it to camp. He had promised her that they would always be there for each other, but by betraying both Olympus and Camp, the broken promise cursed the knife. 

The knife was used by Luke to kill himself.

A single choice shall end his days

Percy's decision on whether or not to give Luke Annabeth's life to destroy himself, or Luke trying to destroy himself and failing, which would mean Kronos managed to take on his true form. 

Olympus to preserve or raze

Percy's choice to allow Luke to kill himself by giving him the blade results in the preservation of Olympus.
