Just a warning that the tutorial may not turn out exactly as the cover above. Also, some drawing is involved. I'll request for you not to remake this cover, but you can use the following steps to make other covers of your own. 

Programs used:

(1) Sketchbook Pro (iPad)

(2) Adobe Photoshop CS6

If you're familiar with Photoshop and know how to do these things more efficiently, then you don't need to follow all of these steps. I'm still fairly new to Photoshop so I don't know all of the shortcuts.

Okay, now to the fun stuff.

(1) The drawing bits

I first drew this cover by hand last year, and it looks like this:

I made the digital cover as a competition entry based on the above cover. 

I designed the title text in three steps that sound simple but can actually be complicated if you don't know what you're doing. XD

(1) Draw the silhouette/outline of the figure. No letters yet, just the head and neck.

(2) Then using the outline, write the text without going out of the outline. This can be a lot of trial and error, so I recommend a lot of patience when you're doing this. XD

(3) Fill in the text with the colour of your choice.

Now, you can do all of the above digitally, but I did it by hand, so I'll walk you through how I made it into a digital file.

(2) Digitize

The first time I made the cover, I re-did it several times. I first attempted to draw it by hand on Sketchbook Pro (on my iPad), and this was the result:
