"Kidney failure?"

"Yes," Yoshimura confirmed. "This type is called rhabdomyolysis where the kidney can erupt from trauma, crash injuries, or burns. I believe it's been slowly deteriorating for a while, but when she was attacked and pinned against the wall, it ruptured and did the rest."

"Can't you just take it out?" Uta stressed, breathing shallow.

Yoshimura got straight to the point. "(Y/N) only has one kidney."

Uta closed his eyes and turned, driving his fist through the wall of the cafe. His chest was heaving up and down and he felt panic for the first time in his life. Options. What could he do?

"Take one of my kidneys," Uta demanded.

"What?" Yoshimura asked, taken aback.

"Take one of my mine," he repeated, shaking from rage and emotions. "I won't lose her."

Yoshimura had never seen Uta that way. Uta was always composed, friendly, and calm, yet he was letting his temper get the best of him. Yoshimura knew he had to do something, but he couldn't morally perform the same operation that turned Kaneki into what he was.

Uta didn't know what else to do. Waiting for a donor would take too long, he realized, and you didn't have much time as it was. He thought of how you didn't care about the way ghouls lived and wondered what it would be like if you lived the same way he did.

"Are you taking her feelings into consideration?" Yoshimura questioned. "She would be half-ghoul. Do you think she would be okay with that?"

Uta couldn't answer for sure. He didn't want to make the decision for you, but what other choice did he have? You were dying and it would surely save you if it was a success. Uta knew Yoshimura could do it and was keen on forcing him if he had to.

"I think she would," Uta muttered, coming to his senses and finding his wits. "I think. There's no guarantee that I'm right, but for as long as I've known (Y/N), she's viewed humans and ghouls as equals. I can't see how she wouldn't allow this since it's our only option."

Yoshimura sighed deeply in defeat. "All right. This is going to take many hours, so get in the other room and prepare yourself while I get what I need."

Uta nodded in thanks and watched Yoshimura descend the stairs. Before Uta went to the other room, he went back to see you one last time prior to getting an organ ripped from his body. Uta crouched next to the bed and took your limp hand in his.

"You're going to be fine," he whispered, his words wavering. "I'm sorry I'm making this decision for you, but I hope you can find it in your heart to forgive me. The life of a ghoul, especially a half-ghoul, is one of difficulty, but I'll do everything to keep you safe."

Uta kissed your hand and continued, "We're going to be even more bonded after this, my love. If--if you don't make it, then I'll join you on the other side because I cannot bear to be without your smile, your laugh, or your being. I love you too much to let you go."

A measly hour later, Uta was down a kidney and it took him only another ten minutes to regenerate back to his old self. Yoshimura had immediately left after performing the incision and retracting the organ to begin operating on you, leaving Uta alone.

He wasn't in pain despite not accepting anesthetic because it would have taken longer. He was up and walking right away and stood outside of the spare room you were in that had the door closed for everyone's benefit. Uta could smell your blood again and he was doing everything to keep himself together.

A mug was suddenly thrust in front of Uta and he over to see Itori and Renji, Itori being the one handing him a coffee. Uta took it and swallowed half of its contents in one gulp before giving it back.

"How are you holding up?" Itori asked.

Uta shrugged.

Itori punched his arm. "Don't pretend you're not freaking out. Talk to us, we're your friends."

Uta clenched his jaw. "What do you want me to say?" he hissed. "My girlfriend might die, but if she makes it out, she might be pissed at me."

"I haven't seen you this agitated since we burned down the wrong building in the fourth," Renji commented.

"That was on you," Uta scowled. "That was another reason why I hated you so much--you were an idiot."

"None of that right now," Itori said.

"Why did you guys come if you were just going to bombard me?" Uta asked.

"Because we love (Y/N) as much as you do," Itori answered sincerely. "When she came into our lives, we didn't know what to think because you'd never taken an interest in anyone, much less a human. We saw how you looked at each other and fell in love with the love you had. We care, too, Uta. Don't pretend we have no right to be here."

Uta bowed his head and couldn't think of what to say. He never realized how much you were cared for by the friends you shared and assumed his protection from everyone was essential.

"She's good for you," Renji added, somewhat out-of-character. "You were a train wreck waiting to crash before meeting (Y/N). We don't blame you for getting crazy over this."

"You can cry if you want, by the way," Itori said jokingly.

To the shock of Itori and Renji, they vaguely caught sight of a dark droplet sliding down Uta's face, but with his head down, it was hard to tell. When Uta wiped a hand across his cheek, it confirmed their suspicions.

Itori reached out and hugged Uta while Renji put a hand on his shoulder. The silence between them rung out until the door to the room you were in began slowly opening.
