And from that moment, you'll be out of place and underdressed  

At fancy hotels, and even some apartment buildings, there is a doorman that you hand your hat and (over)coat to so you can have them handed to you as you leave.

The commands the aforementioned doorman to take away the woman's clothes along with all of the others, thus exposing the vulnerability of her. He purposely wants to make the woman feel uncomfortable – probably as a ploy of revenge.

This is likely to work as she's a woman who hides herself behind all of this fancy outer clothing, so when you take it away, she's left to just have to be herself, which leaves her feeling out of place from these fancier people – something that'd be a nightmare for a self-conscious wealthy lady.

Ruining this banquet for the mildly inspiring and?  

Ryan's got some plans for tonight, as mentioned at the end of the verse  and he's certainly gonna wreak havoc on the event. As the song progresses, he reveals that he's gonna lace a certain woman's cigarettes and add alcohol to the drinks being served.

The "mildly inspiring" are probably the sophisticated people attending the party. They seem to have grace and good manners, which is the "inspiring" part of them, but they haven't really done anything to be considered a true role model. However, they've certainly served as an inspiration for the girl the song is directed at, and she's lost herself while trying to become one of them.

Everything goes according to plan  

  He's dressed very dapper – everything is on point! He's got fancy black slacks, and a white pinstripe; a very narrow stripe in cloth, especially of the type used for formal suits. He knows he looks incredibly fancy, and it gives him the bodacious confidence of the chorus .

  Word about the band is rapidly spreading – much like cancer. Cancer  is a disease of abnormal cell growth. Whether it's you, or someone you know, cancer has seemingly almost effected everyone in some way.  

  The exposure to the world they're currently getting is good, but they can't help but wonder if they're only going to be popular for a short amount of time before they eventually die out. The success they're getting causes much envy of the woman antagonist. 

 Because you say so under your breath

You're reading lips, when did he get all confident?  

Ryan isn't acting like his usual self tonight. He has changed from self-conscious to slightly cocky since the girl has last seen him. He's thriving off of this newfound attention his new self is getting, and he's simultaneously making her jealous – she could have had all that!

The "lip-reading" going on might not be literally watching someone's lips as they talk, but watching their body language and listening to their conversations. Everyone else at the event seem to be impressed by his new persona – it's the kind of positive attention the girl wishes she was getting.

  Next is a trip to the ladies room in vain

It's possible that the girl is under-dressed for the party compared to others in attendance, which was probably planned by the narrator  so she goes to the bathroom in a desperate (and fruitless) attempt to better her appearance.

She may also be overwhelmed by the malicious rumors that are mentioned later in the verse, removing herself from the party in an attempt to regain her composure.

A whispering campaign occurs when rumors about a person are systematically spread, usually in an attempt to damage their reputation. In this case, the other guests at the party are not only spreading rumors about another guest, but also insulting her based on her attire and appearance.

The woman who is the subject of the rumors and insults is aware of the negative comments that others are making about her. In an attempt to boost her self-esteem, she gives herself a pep-talk in the mirror. It's been proven that self-talk can boost confidence levels , although it's more effective when second person pronouns are used, which could explain why she seems unable to convince herself that the rumors aren't true.

  Oh, and the smokes in that cigarette box on table,
They just so happen to be laced with nitroglycerin  

 Nitroglycerin is a medical drug that becomes a deadly explosive in liquid form. It can also have a dangerous reaction to alcohol.

So in fact, the reason the tables are numbered is that the narrator is trying to kill her. By numbering the tables, the narrator is ensuring that the laced cigarettes with be placed next to her, and if she happens to try and light one up, she'll be killed in the explosion.

  And I know, and I know It just doesn't feel like a night out
With no one sizing you up
I've never been so surreptitious,
So of course you'll be distracted when I spike the punch  

It's normal for people to make assumptions about you when you're at a party or a similar event. The first line of the couplet seems like a sympathetic statement said to comfort someone who has been judged by others, but the rest of the outro shows the speaker's true colors. He' sarcastically commiserating with the fact that it must be hard for them to not be the center of attention.

People have been making assumptions about Ryan, assuming he's not very stealthy, or surreptious. However, he's planning on proving those people wrong by displaying just how sneaky he can really be. While everyone is socializing and discreetly judging each other, he's gonna put alcohol into the drinks to make the party more interesting.

What's even more interesting is what this seemingly simple action could do to the woman the song is directed at. As previously mentioned,  he laced the woman's cigarettes with nitroglycerin, which has a potentially dangerous reaction when used with alcohol. If she were to take a sip of the spiked drink, she might feel faint  which would ruin her night out.
