The only thing you need to do is look your opponent in the eye in order to use your quirk.
The side effect of your quirk is that you can get serious headaches if you manipulate someone else's mind for longer than 5 minutes.
You never got accepted into UA because your quirk wasn't "good enough".
You failed the combat exams as you were faced against robots. Creatures without a mind of their own, hence why they are unaffected by your quirk.
