When Natalie rises from her death bed, she is greeted with happiness and love, it's just to her it feels unreal. Natalie feels like she never woken up from her state, maybe it was just for the moment where she felt that it wasn't real life.
After getting weird hallucinations and wicked migraines, Natalie begins to question her reality. As well as begins to question who she really is.
But most importantly, she begins to question if she really did awake from her deadly state.
