In Ketchum, Oklahoma, there was an unfamiliar office that belonged to Dr. Fuller, a psychiatrist. He opened a file from his desk, then revealed Sam, Karina, and Dean seated on chairs in front of him.

He read the file as he pointed out, "You were referred to me by a Dr. Babar in Chicago."

Dean nodded, "That's right."

"Isn't there a children's book about an elephant named Babar?"

Dean opened his mouth to state a reason but Karina interrupted him in her brown blazer and skirt with her hair into a bun. "Dr. Fuller, I know it is hard to believe but Dr. Babar and I have made connections in the past five years. I had been taking care of my patients, Alex and Eddie, due to their circumstances that haven't been quite cured. Dr. Babar let me contact you due to your brilliant work for the patients. I hope you would do the same with them as well."

Dean inferred, "Look, doctor, I-I think the doc was in over his head with this one." He pointed at Sam, "'Cause my brother is..." He circled his finger at the side of his head then whistled.

Dr. Fuller raised his hand to quiet him down, "Okay, fine, thank you. That's-that's really not necessary." He grabbed a file and the notepad with his pen as he faced Sam, "Why don't you tell me how you're feeling, Alex?"

"I'm fine. I mean, okay, a little depressed, I guess."

He recorded what Sam said, "Okay. Any idea why?"

"Probably because I started the Apocalypse."

He looked at Sam as he repeated, "The Apocalypse?"

He nodded shakily, "Yeah, that's right."

He looked at Dean who faked smiled at him then toward Karina. She sighed hopelessly, sharing eyes with him. He looked at Sam as he asked, "And you think you started it."

"Well, yeah, I mean... I killed this demon--Lilith-- and I accidently freed Lucifer from Hell. So, now he's topside, and we're trying to stop him."

"Who is?"

"Me. A-and them. Debbie, she is our Huntress who had to make a decision to choose a side between me and my brother." Dr. Fuller looked at Karina who shrugged her shoulders, helpless. "Also, this one Angel."

Dr. Fuller pointed it out, "Oh, you mean like a-- like an angel on your shoulder?"

"No, no. His name's Castiel. He wears a trench coat and he's her Guardian Angel."

Dr. Fuller looked down on his notepad then Dean explained little more, "See what I mean, doc? I mean, the kid's been beating himself up about this for months. The Apocalypse wasn't his fault."

Dr. Fuller looked at Dean, stunned. "It's not?"

"No. There was this other demon--Ruby. She got him addicted to demon blood. Near the end, he was practically chugging the stuff." Dr. Fuller looked at Sam who was ashamed. He returned back to Dean, "My brother's not evil. He was just... high. So, could you fix him up so we can get back to traveling around the country and hunting monsters like the Dynamic Trio we are?"

Dr. Fuller raised a finger for them to wait, then he picked a phone. He dialed to connect to the secretary, "Erma... cancel my lunch." He hung up a call then he leaned over to Karina, "Miss Debbie Harry, I'm wondering if you're willing to accompany me in these circumstances. It would be nice that your patients will enjoy the connection due to your history."

"Of course, I would be happy to. Nothing gives me great pleasure to take care of my boys." Karina laid her hands on Sam and Dean laps comfortably as she smiled at Dr. Fuller.
