Explanation

So here is a short explanation just in case people got confused.

Everything that happened before the crash in the park was real. Everything after that, leading up to when she woke up in the hospital was not. So them painting each other, her and Liam kissing, and Harry dying was all in her head. The only thing that was real was Louis and Harry having sex in the hospital.

Sandy was not in a coma. It may seem like it but I don't want to say that because to me she wasn't. She could have woken up any time but she didn't. She was afraid of what she would wake up to. Everything she experienced in her sleep was her trying to deal with her struggles. She was in essence "trapped in her own thoughts" and had to deal with them before she woke up.

When she woke up she was stronger and would now be able to handle her anxiety better and soon she would be okay.
