It looks like the fight will never end between Sam Winchester and the devil himself, Lucifer.

My nose was filled with the smell of disinfectant and fresh sheets. There was a small draft of cool air in my nose as well. I'm in a hospital. The faint beep of a heart monitor and muffled voices filled my ears, but I could not open my eyes, nor could I move my leg and arms, hands or feet. Was I paralyzed? No, finally one of my fingers shifted, I could barely hear the quiet breathing beside me.

"Sam, I don't know if you can hear me but-a, but you gotta come back man. I don't really even know what happened to you. Cas...Cas has done everything that he can do for you." Dean's voice came out uneven and shaky. I tired to open my eyes but I couldn't, I didn't want to hear Dean say anymore, I wanted to sit up and tell him I was fine. "Sammy, y-you've been out for a month now, I've talked at you too many times man, I....I can't just sit here and watch you...." There was a catch in his voice, he was starting to cry. No Dean, please don't, I'm not dying, I just can't wake up.
"I can't just sit here and watch you waste away. I don't know how Lucifer did this but when I get my hands on that son of a bitch, he'll wish he NEVER set foot out of that cage!" He yelled through his tears. I heard him throw a chair, a moment later a nurse rushed in. "I'm sorry, everything's fine." I heard Dean say to the nurse, the nurse was not happy with him, apparently this has happened before. Warm tears weld in my closed eyes, Dean grabbed the chair and came back to sit with me. A tear started to crawl it's way down my cheek. "Sammy? Can-can you hear me?" Dean must have seen the tear, I could feel him wiping it off my cheek. I need to do something but what? I tried to move my hands, it felt like there was a brick tied to them, I finally managed to lift my left hand, pointer finger. My heart monitor skyrocketed as I did so. "Sammy! Oh it is good to see some life in you, man!" Dean said with relief. Seconds later the nurse came back in and my heart monitor went back to normal. "What's going on here?" She demanded. "He uh moved his hand, when I asked if he could hear me!" I could hear the excitement in his voice. "He might have moved his hand but that doesn't necessarily mean he can hear you, Mr. Winchester." At that moment I realized that there was a feeding tube down my throat, I started choking on it, all sorts of machines went off. The beeping was overwhelming and the commotion going on around me was suffocating, my eye lids shot open. "Sammy! Calm down, its alright!" Dean said as I started pushing the nurses and doctors away from me. Panicked, I grabbed the feeding tube and pulled on it.
"Mr. Winchester don't!" One of the nurses yelled as I pulled the tube out of my throat. I gasped as I threw the damned tube, pushing everyone off of me, I sat up, taking in a huge breath of air. I started to rip all the Iv's and other tube's off of me. "Mr. Winchester please settle down! I know this is a lot to take in but you need to slow down!" The doctor yelled as he grabbed my shoulders. Forcing me to look at him, the room fell silent, my gaze shifted towards Dean. "It's good to see you, brother." My voice came out uneven and hoarse. The doctor let go of me. "You too man, you too." Dean said with a smirk, pushing the doctor to the side, we hugged each other tightly. I was afraid to close to my eyes, fearing that if I did I would find that I was still unconscious and was imagining the whole thing.
After a screaming match and millions of signed paper work, Dean and I were sitting in the impala in the hospital parking lot. "Was I really out for a month?" I asked Dean as he started the car. "Yeah, no one knew what was wrong with you, your wrist was broken, I guess it healed up pretty nice." "Dean, cut the small talk and tell me what the hell is going on." I demanded. After what seemed like forever, Dean finally looked me in the eye. "After what happened to you, I checked in with Crowley and...." "And what?" "Lucifer's...gone and the cage, the cage is destroyed." "What?!" "He's out Sammy, and he said he's back for you." Suddenly I couldn't breathe, I became so dizzy, I pushed open the car door and threw up on the pavement. "Damn it Sammy!" Dean got out of the car and walked over to me. He helped me out of the car, over my vomit and walked me over to a little island to puke in the grass. I vomited a couple more times then sat down in the grass. "Dean - I-Ahh!" I yelped as my head exploded with pain. I doubled over, hands on my head, gasping for air. "Sam!" Dean grabbed me by the shoulders, I could hear in his voice, he felt helpless. A face started to flash in front of me, blurry at first, then clearer as the pain got worse. The face belonged to a woman, her skin was pale and her hair was dark. Her face was slender and her features soft. Her eyes were strange, changing a shade of purple everytime she or I blinked.

The pain became more and more intense. "It's alright Sam." Her voice echoed in my head, her voice was gentle and familiar. "You will soon understand, everything will become clear once we meet. I'll see you in Detroit." Just like it had never happened, she faded away and the pain was gone. Confused I looked up at Dean. "What the hell was that?" Dean looked concerned and pissed. "Dean, we got a problem." I started, but stopped when dark clouds raced across the sky, blocking out the sun, all the street lamps came on as it became as dark as night.

"Okay, now what the hell is that?!" Dean asked as he helped me to my feet. "Belladonna." I whispered as we stared at the darkened sky. People walked out of buildings, car stopped, everyone stood, staring. Then all the lights went out and we were all left standing, wondering in the dark.
