One of the wizards who made the barrier were blind, so they made it so that anybody who lives in the Ungrounded will start to have vision problems. Whether they're blind, missing an eye, colorblind, etc.
This is how I'd imagine it being. I'm not going to base all of these on any actual vision problems.
Frisk - When they fall, things slowly start to be seen in inverted color.
Flowey/Asriel - He is nearsighted. (Vision is perfect when he absorbs souls.)
Toriel - She is nearsighted.
Sans - He can only see the color blue; everything else is black, white, or gray
Papyrus - He can only see the color orange; everything else is black, white, or gray
Undyne - She can only see things that are standing in place and can only see out of her right eye.
Alphys - She is just plain blind but can see things better through a screen. (Like a TV.)
Asgore - He is nearsighted.
