I was thinking about how you could have literally anything under your mask since no one would see it, so my idea was "what if I drew blood dripping from my mouth?"

I found some old lipstick of mine (a dark shade of red) and drew squiggly lines down the corners of my mouth. Then I got fancy and got a darker red lip liner and outlined one side to act as the "shadow". Sadly there's no highlight since I have no white lip liners or anything.
Again, no one saw this luckily, otherwise I'd have some serious explaining to do.
