Shigaraki is 19 and Natsuo is 18 and a doctor. No one knows that Izuku(Deku) and Shigaraki(Tenko) are siblings not even their significant others know until a surprise visit. Their significant others know they have a brother but don't know who the brother is.
And no one knows about Toga x Bakugou siblings and Dabi x Todoroki siblings it is the same rule as Deku x Shigaraki siblings except Shigaraki knows the Dabi is Touya and that Touya is Natsuo brother.
Tenko and Natsuo are secretly dating but Touya is the only one that knows they are dating.
Izuku's nickname for Tenko is Blue Bunny Boo. Tenko's nickname for Izuku is Green Baby Bear. They had those nicknames for each other since childhood.
Izuku and Bakugou are on the roof eating lunch. While on the roof Izuku text Tenko

"Deku who are you texting~," Kacchan asked looking at my phone.
"My older brother," I said rolling my eyes playfully. "He had a boyfriend but he won't tell me who it is," I said fake pouting. "Why don't you trick him into answering some of your questions about him," Kacchan suggested. "Oh yeah, that could work!" I say excitedly. We both look at my phone.

Kacchan and I were laughing our asses off at Tenko's response we didn't knowns the BakuDeku Squad coming towards us. "What are you guys laughing about?" Ocho~chan asked. "Oh, my brother that's all!" I said with an innocent smile. "What you have a brother!?" The whole BakuDeku Squad yelled in shock. "Well yeah, I just keep having a brother secret. Like Kacchan having a sister secret." I said. They all looked at us shocked. "Anyways my brother has a secret boyfriend and he won't tell me who it is!" I continue. "Do you have any info about the boyfriend?" Mina~chan asked. "Yes, actually he's-" before I got to finish a portal appeared behind them. Tenko came out of the portal I came running to the portal to hug him. "Bunny Boo!" I Yelled hugging him. He hugged me back. "Hey, Baby Bear. I thought it would be a good time to visit." Tenko said with a smile.
Kacchan walked to us from behind. "So your Izuku's brother it is a pleasure to meet you properly Shigaraki!" Kacchan said with a smile sticking out his hand. "You don't have to call me Shigaraki call me Tenko instead! And it is a pleasure to meet you properly as well Katsuki!" Tenko said with a smile shaking Kacchan's hand. They let go of hands. "Oh, I remember why I came here in the first place! Baby Bear remember our conversation about my boyfriend?" Tenko asked me. "Yes yes, I do!" I said excitedly. "Well, I would like you to meet him if that's ok with you?" Tenko said/asked. "I would love to!" I said. "Ok let me get him!" Tenko said. Tenko walked back into the portal and came back with a tall white-haired man. "Hello, my name is-" the tall white-haired man was cut off by Todo~Kun. Todo~Kun was running towards us. "Natsuo!? What the hell is going on?!" Todo~Kun yelled with a coufused look on his face. "Oh, hey Sho!..." The tall White-haired man said with an awkward wave and an awkward smile to Todo~Kun. "When were you planning to tell me this!?" Todo~Kun shouted with tears in his eyes. "Sho I'm sorry for not telling you I know you're upset just please calm down!" The tall White-haired man other known as Natsuo said trying to get closer to Todo~Kun. "Natsuo you lied to me! Your turning into Touya! Had you ever told me the truth!?" Todo~Kun yelled with more tears running off the roof. "Sho wait!" Natsuo yelled with tears down his face. He looked a Tenko, Tenko smiled then nodded. Then he ran down the stairs.
No one Pov:
What everyone didn't know was that someone sneaked downstairs unnoticed.
Natsuo Pov:
He couldn't get far I have to find him before something happens. I thought as I run through the halls. I got to the first floor and found Sho sitting on the floor with Iida his boyfriend. "Sho please let me talk to you!" I said with tears down my face. He nods and I run to him. "Sho I-" Shoto cut me off. "I am sorry Natsu for my behavior it was uncalled for me to talk badly about you and Touya. I was just so fed up with you two keeping secrets for me but I realized you had your reasons I just hope you will forgive me!" Shoto said with tears down his face. I then hugged Shoto. "Of course, I forgive you it wasn't your fault it was mine and Touya's for keeping secrets from you. We should have told you I'm sorry." I said with tears down my face. "Love you Natsu!" Shoto said. "I love you too Sho!" I said back.
We stopped hugging I turned to Iida and hugged him. "Thank you for taking care of Sho! I know he loves you a lot so good luck!" I whispered into his ear with a smile. "Thank you!" Iida whispered with a smile. They let go of the hug. "Let's go back!" I said. They both nodded and walked with me.
On The Roof at the same time with Tenko's Pov:
Izuku's friends were being nice to me asking questions as I answer them.

Then Puppy, Shoto, and Shoto's boyfriend came back on the roof. I ran to Puppy and hugged him. "Hey Puppy how did it go?" I asked. "It went well!" Puppy said with a smile. "Now that's over we can finish introducing everyone!" Puppy said as we walked over to Izuku and Katsuki. "Now where was oh yes my name Natsuo Todoroki it is a pleasure to meet you both Izuku Midoriya and Katsuki Bakugo!" Puppy said with a smile shaking both of their hands.
"It is a pleasure to meet you as well but is Todo~Kun okay?" Izuku asked a little worried. "I'm fine Midoriya I was just a little shocked that's all," Shoto said in a calm voice. "IcyHot you ran off the roof crying obviously you're not okay!" Katsuki said worriedly. "I said I Was Fine!" Shoto screamed with tears down his face. Puppy picked him up and hugged him.
Natsuo Pov:
I picked Sho up as he hugged me falling asleep on me. I carry him to a bench as I sit down with him on me. Everyone followed. I pet Sho's hair as I talk. "I'm sorry for him screaming at you it's just that the reason why he cried before was that he is sensitive about that topic," I said. "Who is that Touya person he talked about? If it's not too much of a sensitive topic to talk about?" A boy with red spiking hair asked. I sighed. "No that's fine. Touya is our oldest brother he was presumed dead a long time ago. But recently Sho and our family minus me found out that he was alive. Since then Sho found out that I knew Touya was alive before everyone else did. Sho considered that like me and Touya lying to him. Since we keep it from him for a long time. Both me and Touya had secrets we keep from everyone and Sho hated it. He couldn't tell if we were telling the truth or lying to him so he built up anger at us. Since our jackass of a father made us hide our emotions. This made Shoto hide his emotions from everyone making him emotionless. But what happened today made all of the emotions that Shoto had been hiding for years come out at once making him fall asleep unconscious. Any more questions?" I said/asked in a serious and slightly happy tone. Everyone nodded no. I kissed Sho on the head then I got up. "It was lovely meeting you all but I have to go to work, please take Sho to the nurse and let Recovery Girl take care of him. When he wakes up don't say anything about any of this and go soft on him for now so he won't pass out. K bye!" I said as I and Kitten waved goodbye walked into the portal.
