The transcript that was written is based from the doctor's own recordings in courtesy of her patient's journals that are labeled as P-16. Yet, somehow, the recorder has choppy lines and even the writings in some pages as well are either blurry or has a smudge of an ink and dried blood on it. So, don't blame the author if some circumstances represent their best guesses.
Out of respect for the dead, names have been changed. Some of the events, dialogue, and chronology have been fictionalized for dramatic purposes.
