Derek woke up to his ears ringing and people screaming. He slowly opened his eyes. Their plane crashed. Literallt crashed. And they were in the middle of nowhere.
"Derek, you good?" Mark asked as he limped over to his best friend. "Yeah, yeah. Are you?" Derek asked as he came to his senses.
"I think so. Uh, have you found anyone or are you just coming around?" he asked. "I don't know. I just woke up." Derek answered, wincing as he stood up.
"Have you found anyone? Meredith?" Derek asked. "No. Arizona and Cristina are both okay, but they both have bad broken bones. I haven't found lexie or Meredith yet, though." Mark said.
"We need- we need to find them." Derek said, swallowing thickly at the thought of his wife being hurt.
"We're gonna go look for the other two. Are you guys okay here?" Mark asked as they walked to the other girls. "Yeah." Arizona and Cristina responded.
The two guys trudged through the woods, both needing desperately to find their Grey girls. "You don't think they're....." Derek trailed off. He was terrified of what state he would find Meredith In.
"I don't know. I hope not." Mark replied, his voice slightly shaky. "Meredith! Lexie!" He called out. "Mark!" A voice yelled back.
"Lexie. Where are you!" Mark said as he and Derek began jogging until they finally came upon her.
Derek's heart dropped when he saw his wife, leaning up against Lexie. Her head was a bloody mess, and she looked like she was fighting to stay conscious.
"I wanted to find you guys but I couldn't leave her and I didnt think she should walk." Lexie explained. "Mer..." Derek breathed, dropping to his knees and looking at her.
Meredith winced as she opened her eyes slowly. "She can't really talk. She definitely has a brain bleed." Lexie said nervously.
"Meredith, it's Derek. Can you hear me?" Derek asked, cupping her cheeks. Meredith looked at him with unfocused eyes, and through her blurry vision, she made out the form of Derek.
Meredith nodded slowly. "Can you try to say something for me?" He asked. Meredith opened her mouth to speak, but nothing came out. Derek gulped.
"Okay. You're okay." He said, reaching into his pocket to see if the penlight he carried was still in one piece. Thankfully, it was.
He shined the light into her eyes and she whimpered in pain. "Her left pupil is blown." Derek said in hushed tone to lexie and mark.
"Crap." Lexie breathed. "If I don't relieve the pressure, she'll die." Derek added in a shaky voice.
"We should get her back to the others. Maybe there's something we can find to do burr holes." Mark suggested. "Okay. Mer, can you stand up?" Derek asked gently.
Meredith looked at him and shrugged. Derek carefully pulled her onto her feet. "Okay?" He asked. Meredith nodded.
Mark and lexie walked ahead while Derek helped Meredith along with a hand around her waist, holding her up.
That was, until, Meredith stopped and began throwing up. "Okay, you're okay. You're okay." Derek said soothingly, rubbing her back.
Meredith stood up dizzily and she knew she couldn't walk anymore. Derek kept her steady. "Can you keep walking?" He asked. Meredith shook her head weakly.
He picked her up by the waist and she wrapped her legs around him and her arms around his neck. Thankfully, his wife was a very small person, so it wasn't a problem to carry her.
Merediths head was throbbing and it was getting really hard to not be able to talk. She could think and understand and everything, but she just couldn't get the words to leave her mouth.
They got back to their 'camp' and Derek gently put Meredith on the ground, leaning against a piece of the plane next to Cristina.
"Is she okay?" Cristina asked in a worried voice. "Her pupils completely blown and I have nothing to relieve the pressure with." Derek explained.
"When she falls asleep, I need to do it or else she won't wake up." He said.
"Wait. Use this." Lexie said suddenly, reaching into her coat pocket and pulling out a tall tube of hand sanitizer. Derek nodded and unscrewed the cap.
"This could work. Anyone have a knife or sometning?" Derek asked hopefully. "Always do." Mark replied, tossing mark the pocket knife he always kept on him.
Derek looked over to his wife, who was asleep. "Mark, lexie, come over here and make sure she doesn't wake up or move or anything. Cristina, can you just watch her pulse?" Derek asked. She nodded and put her fingers on Meredith's neck.
He took a deep breath before cutting one small hole in his wife's scalp. He stayed calm when the blood oozed out, reminding himself that this was completely normal for patients with brain bleeds.
Derek was about to make his second burr hole when he heard something. "Look!" Derek exclaimed, seeinf the helicopter.
Maybe they all wood survive after all.

The next thing Meredith remembered seeing was the blurry vision of her husband. Now, she slowly opened her eyes, hearing the rhythmic beeping of the machines next to her.
Her head still was throbbing, but not in the way it was in the woods. "Welcome back." Dereks tired voice said.
Meredith looked at him and smiled slightly. "H-hi." She whispered. "How are you feelin?" He asked. "Head....hurts. Slow." Meredith replied, her speech sounding a little funny, which was normal after a big brain bleed.
"That's okay. Do you need pain meds?" He asked, putting his hand on her cheek and stroking it gently. "No. Long....how long.....how long I sleep?" Meredith asked, frowning when she couldn't speak correctly.
"Don't worry about your speech now. It's normal after a brain bleed. You might need some speech therapy, but it's okay. And you were asleep for almost 4 days after they rescued us." Derek explained.
"Hmm." Meredith hummed. "You don't have to talk now." He said quietly as he carefully climbed into the bed beside her.
"I love you." Meredith whispered. "I love you too." Derek replied with a soft smile.
Just a take on how the plane crash could have gone
