Dr. Derek Shepherd, a renowned neurosurgeon, and his wife, Dr. Meredith Grey, shared their home with Derek's siblings and their significant others. Mark Sloan-Shepherd, raised as a Shepherd and Derek's brother in every sense, was engaged to Lexie Grey, Meredith's half-sister. Mary Shepherd, the youngest of the Shepherd siblings, was a prodigious second-year resident at Seattle Grace Hospital, where her brothers also worked. The tragic loss of their parents had brought the siblings closer, especially Derek and Mark, who became Mary's guardians after their mother's death.

On this particular evening, the living room of the Shepherd house was abuzz with medical jargon and lighthearted banter. Mark, Lexie, Meredith, and Jackson Avery, Mary's boyfriend, were engaged in animated conversations about their respective surgeries of the day.

"So, I had this patient today," Meredith began, her eyes gleaming with the thrill of the challenge, "with a rare hepatic tumor. It was benign, but took Richard over 12 hours to remove."

"That's great, Mer," Lexie chimed in, her tone enthusiastic. "I assisted Mark in a face reconstruction surgery. It's amazing how much difference it can make in someone's life."

Mark, leaning back with a relaxed posture, added, "It's always rewarding to see the immediate impact of our work. The kid was thrilled, even with all the swelling."

Jackson nodded in agreement, his gaze shifting occasionally towards the hallway, wondering about Mary's absence. "These surgeries can really change lives. It's why we do what we do, right?"

Their conversation, a blend of professional pride and personal connection, was abruptly interrupted by the sounds of shouting coming from Derek and Mark's office. The voices were muffled, but the tone was unmistakably tense. Derek and Mary had arrived home an hour earlier and had immediately sequestered themselves in the study for what appeared to be a serious discussion.

The family in the living room exchanged concerned glances, the atmosphere thickening with unspoken questions. After a few moments, the shouting subsided, replaced by a heavy silence that stretched into minutes. Then, the study door opened, and Mary emerged.

Her eyes were red, betraying her recent tears, and her movements were hesitant, almost as though she was in discomfort. The sight confused Jackson, who hadn't witnessed such a scene in the Shepherd household before. Mark, however, seemed to understand the situation and sighed, a mixture of empathy and resignation in his expression.

As Mary approached, Mark extended a pillow to her, an unspoken gesture of care and support. She gratefully accepted it, settling next to him on the couch and cuddling into his side. There was a protective quality in the way Mark wrapped an arm around his sister, offering her silent comfort.

The rest of the room watched the scene unfold with varying degrees of understanding and concern. Derek soon followed his sister into the living room, his expression stern and authoritative. Mary visibly shrank under his gaze, nestling even closer to Mark, avoiding eye contact with her eldest brother.

Derek took a seat next to Meredith, his posture rigid, a clear sign of the lingering tension. The room was heavy with unspoken words and unasked questions, the atmosphere distinctly different from the light-heartedness that had prevailed just moments ago.

In an attempt to dissipate the sudden awkwardness, Lexie, ever the peacemaker, restarted their earlier conversation about the surgeries. Her voice was bright, but her eyes held a hint of concern as she glanced towards Mary.
