Silence. That's all there was.
The air hummed with tension as Castiel obviously waited for them to do just that. Bow down and profess their love unto him their new God. Then Bobby slowly got to his knees. "Well, alright then. Is this good, or do you want the whole forehead-to-the-carpet thing?" Looking over at the Winchesters and Alayna, he prompted, "Guys?"
The Winchesters slowly knelt, and Alayna frowned indignantly at the sight. "Cas!" She stepped forward. "Stop this! Put the souls back!"
"Hush, now!" Castiel simply raised a hand and no matter how hard Alayna tried she couldn't get the words in her head to became vocal. She scowled in frustration at the Angel. He looked at the other three hunters, a frown on his face. "Stop. What's the point if you don't mean it? You fear me. Not love, not respect, just fear."
Sam slowly got up. "Cas..."
Castiel cut him off. "Sam, you have nothing to say to me. You stabbed me in the back." He looked down at Dean and Bobby, obvious annoyance flickering in his dark blue eyes. "Get up."
"Cas, this isn't you." Dean stepped forward, his green eyes desperate.
"The Castiel you knew is gone." Castiel cut him off, face blank.
"So, what, then?" Dean challenged with a light scoff. "Kill us?"
"What a brave little ant you are." Castiel walked back over to the Winchester. "You know you're powerless; you wouldn't dare move against me. That would be pointless. So I have no need to kill you. Not now. Besides... once you were my favorite pets before you turned and bit me."
"Who are you?"
"I'm God. And if you stay in your place, you may live in my kingdom. If you rise up, I will strike you down. Not doing so well, are you Sam?" Castiel turned his head to look at the Winchester, who was swaying, unsteady on his feet.
"I'm... fine." Sam cleared his throat in an attempt to steady his shaking voice. "I'm fine." He straightened
Dean stepped forward. "You said you would fix him — you promised!"
"If you stood down, which you hardly did. Be thankful for my mercy." Castiel looked back at Sam. "I could have cast you back down in the pit."
"Come on, Cas, this is nuts. You can turn this around, please!"
"I hope for your sake that this is the last time you see me." Was the last thing Castiel said before disappearing.
As soon as he was gone Dean ran over to where Karoline laid crumpled on the floor, wide, panicked, green eyes taking in her relaxed face as he flipped her over. "Come on, Kare. Wake up!" He shook her shoulders, but she never moved. His hands moved up from her shoulders to her neck, checking for a pulse. He let out a heavy sigh at the pulse beating slowly against his fingers. "She's alive." Dean announced and Alayna nodded, raising a hand up to push her hair out of her face as she felt this weight lift off her shoulders.
Sam swayed even more, and Alayna started. "Sam?" The hunter fell to the ground, shaking, eyes wide, and Alayna was at his side in an instant. "Sam!"
"Sam?" Dean ran over to his fallen brother, who was gasping, unable to form words. His eyes rolled into the back of his head and he collapsed into Alayna's lap. "Sam!"
"Come on, guys." Bobby rushed over to them. "Dean, you get Karoline. I'll help Alayna with Sam." Dean looked up at the older hunter before turning worried eyes down on his little brother - after a moment he nodded, pushing away to go scoop up the unconscious blonde.

Blair walked out to the junkyard that Dean seemed to be holed up in for the last week. Karoline and Sam were still out of it - neither showing any signs of waking up. The only difference between them was when Blair entered Sam's subconscious she was filled with visions of fire and pain, but when she did the same with Karoline it was nothingness.
