How To Unlock
In order to get this ending there are a number of things you must do:
1. Once you have Angela's knife, make sure you examine it in your inventory at least once.
2. You have to read the diary on the roof of the hospital.
3. You have to listen to the audio recording in the hotel.
4. In the hallway right before the final boss fight, make sure you hang around to hear the whole conversation.
5. Throughout the game, do not heal yourself immediately if injured. Spend as much time at low health as you feel comfortable with.
Follow these rules and you should manage to get this ending.
