How To Unlock
There are several things required to get this ending, most of which have to do with how attentive you are to Maria throughout the game.
1. While Maria is following you, do not lead her in the wrong direction. If at any point she says something like "Where are you going?" you might as well load your last save.
2. Do not allow her to take damage from enemies, and especially do not injure her yourself.
3. When she falls ill in the hospital, make sure you return to check on her several times. Though she's not in the room in the "Other World" it's a good idea to visit it and examine the empty pill bottles anyway.
Description
James realizes he doesn't need Mary. He goes to Rosewater Park and find Maria. He decides to leave town with her. Just as they head to the car, Maria starts coughing like Mary did.
