Hospital Day-time,A Doctor is working at his desk Dean opens the door and enters, followed by Sarah a nurse and the Nurse to the both of them "Okay Sir, Madam? You both can't just just barge in here without an appointment!" Dean angrily but trying to keep his calm asks the Nurse "They said, "Talk to Kandinsky." You Kandinsky?" The Nurse ignores he's question and says "You need to be scheduled!" Dean then says "Well, then, schedule us! He was in a car crash so why, the hell can't we see him?" Dr Kandinsky then walks over and asks him "You're Sam Smith's brother and Friend." Dean says "Yeah, what's going on?"
But before the Doctor answers Dean's question she turns to the nurse and says "It – it's fine thank you, really." The Nurse leaves and the Doctor looks to both Sarah and Dean, she says "Sam was admitted and he was treated for a broken rib and lacerations." Sarah says "Okay, that's not too bad. And?" The Doctor responses with "And... he's on our locked psychiatric floor." Sarah and Dean both look at each other surprised by what the doctor had just said, Dean finally spoke and says "I mean, he's had some trouble..." But before Dean could finish the Doctor asks the both of them "So you're both aware that Sam is experiencing a full-blown psychotic episode?"
Dean says to the Doctor "Psychotic? Come on. I mean, the guy's... It's not like the guy's freakin' Norman Bates." The Doctor says to the both of them "No, I'm sure he isn't but we need to determine whether his state was brought on by the insomnia or whether the insomnia is a symptom of his condition. Do you both understand? So that we can figure out how to treat him." Dean was about to say something then Sarah says "Well, all I can say is that the sleep thing is – is kind of new." Dr Kandinsky says "Right. Well... we've pumped him about as full of sedatives as we safely can. So far, he won't go under I've never seen anything like it."

The three of them are at the lift and Doctor Kandinsky presses the button to enter Ward D2, A sign on the wall and the door read "High Security Area". He, Sarah and Dean walk down the hallway in the ward. Inside Sam's hospital room Hallucifer is sitting on a desk, playing with a piece of string and Sam is on the bed, dressed in a white T-shirt and white hospital pants Hallucifer says Sam "I'm just sayin', Back when you had no soul..." But before he could finish the sentence Dr Kandinsky, Sarah and Dean appear at the door "...you never had to sleep." Both Sarah and Dean enter the room and Hallucifer continues with "Ah, Mr. Helpless and his beloved girlfriend Miss. Helpless please pull a six-pack buddies."
