Cas was being forced to make a choice. He decided to take Sam's hallucinations of Lucifer again for the 2nd time.
Sam begged him not to. It was selfless enough to do it once, let alone do it twice. They had no idea how he just suddenly stopped being crazy. They always just figured that it was whenever Naomi mind-hacked him. It was a trade. Your sanity replaced for you to be my personnel pawn. And they were sure as hell not doing that again. No way no how.
But Sam's wall somehow crumbled again, and this time it was not even the angel's fault. But he still wanted to take them. "I Am doing this for you.'' He promised Sam.
And so it was. Sam and Dean would certainly figure out how to get Lucifer out.
