Cas needed surgery. He was complaining about severe pain on his bottom right side, and Sam said that it was probably appendicitis, so Cas shold have an emergency surgery right away. Cas did not want to but Sam insisted that it was for his own good and that he needed to be monitored afterward during recovery.
And so, it was determined at the hospital that Cas did indeed have appendicitis, and his appendix was going to burst soon. Luckily, they gotten it out just in time. Cas was scared, but the surgery went well. He awoken in the hospital bed, sore and groggy. He stared at his stitches, looking at his forever scar.
Once he was allowed to leave the hospital, Sam and Dean monitored him, taking turns and watching over him to help him recover quickly. After a few weeks, he fully recovered and went back to hunting with the Winchesters.
