Tom: "Sir, I think I'm slowly dying."

okay okay let me tell you how I got this

so

one day I needed a cough-drop, so I went to the nurse, and in the reason section I wrote "Cough-drop". Then something happened at lunch and I came back, and that time I wrote "slowly dying".

The next day my friend said she saw me write my name twice, and when she looked at the reasons she just went: "What was in that cough-drop?!"
