The protagonist thinks they hallucinate. Because obviously the fantastical creatures and squadrons of heroes can't be real. Except for one day they discover they're not actually hallucinating, and there's actually a huge game (like a video game or something) going on, and they're supposed to be an NPC. For some reason, though, they can influence the game and have a mind of their own. They could use this knowledge to either help a certain group of players win the game or take down the evil people running it.
