1) I headcannon that a group of illegal scientist's had taken him as a child as the mother had sold him off to pay for a drug addiction she had. So he hadn't grown up with sane people as such.
2) His eyes are rotten that's why they are black, they fed him Colloidal silver and this turned his skin grey not to mention the countless other tests they preformed on him.
3) The only food he had was organs, so as soon as he escaped he began hunting people down though it was mainly kidneys. A cannibal from birth basically.
Oh and bonus fact:
4) His insanity was produced because of his isolation, causing hallucinations etc as when he wasn't tested on he was locked in a room with static played from some hidden speakers.
