The best thing for Sam is to lock him up in Bobby's panic room where nothing can touch him. As soon as Castiel knocked the wall down, Sam had completely passed out. He is stuck in his own head, but the worst thing is that Dean doesn't know what is going on up there or even how to help. The only good thing to come from this is that you haven't called him to let him know his daughter is coming. He can't leave Sam knowing he is in a world of pain up there–if he even is.
"Sammy? Come on, snap out of it," Dean pleads as he sits next to his brother's body.
Dean spends an hour... two... four... inside the panic room, but Sam doesn't wake up. Bobby joins the older brother knowing they have to move on if they want to stop Crowley and Castiel from opening purgatory.
"Anything?" Bobby asks.
"I can't just sit here, Bobby. I've got to help him."
"Dean–"
"You know, dreamscape his noggin. Something."
"You know what Cas did. The dam inside your brother's head is gone, and all hell's spilling loose. We don't know what's going on inside."
"I don't care. We have got to do something!" Dean yells.
"And we will, but we got sixteen hours until they pop open Purgatory. My daughter is in the hospital about to have your kid, and we can't count on anyone else right now. We are down two people, and I can't afford to be down one more."
"Yeah, and how's that going, huh? We've got no line on Crowley, we got no line on Cas, and Balthazar's MIA. I mean, all we've got is Sam going through whatever the hell this is!"
"You know, this is exactly what Castiel wants," Bobby says grimly. "For you to fall to pieces. Just try to think of what Sam would want."
"Find Cas, Bobby. Find him now," Dean growls.
"I will, and here is how we're going to do it. Nothing can be done for Sam right now. He has to pull through on his own, and you know it. I can take care of Purgatory and those bitches, but you have somewhere to be."
"Bobby, I can't leave you alone with this."
"And you can't leave her alone with that. She needs you right now. Look, I found this a while back, but I think you need it more than I do."
Bobby unfolds the napkin he pulled from his jacket pocket and reveals the promise ring Dean gave to you. Dean takes one look at it and remembers the exact moment he gave it to you.
"Hey, so, I have something for you," he said nervously.
Looking at him, you wondered what he could be so nervous about.
"What is it?" you took another bite.
"Open it," he said as he slid you a ring box.
Taking a few moments to stare at it, you looked at Dean before swallowing your food. Setting your chopsticks down, you grabbed the box before slowly opening it. Staring at you right in the face was a simple yet elegant diamond ring. The diamond was small, but you knew what it represented. Gasping lightly, you looked at Dean so he could explain.
"You said my ring was too big for you," he said as he grabbed the ring from the box before grabbing your right hand. "I was serious when I said this ring would represent my promise to always love you. You said to get a real one, and I did."
"Dean, this is beautiful," you whispered as you stared at the ring. "Where did you get this?"
"Pawn shop," he chuckled. Knowing he got it illegally, you didn't say anything else about it after that.
