Annabeth, Tyson, Grover, Helena, and Percy go into the labyrinth.

The dead, the traitor, and the lost one raise

Nico raises the dead, Levi officially joins Kronos, and Grover finds Pan. (The dead, the traitor, and the lost one, respectively.)

You shall rise or fall by the ghost king's hand

Nico calls himself the Ghost King during the battle in Daedalus' workshop, and he chooses to save Percy, Helena, Ethan, Mark, and Annabeth's lives.

The child of Athena's final stand

Daedalus, a child of Athena, makes a decision on whose side to be on.

Favoured daughter of war will aid in their journey

Helena is the daughter of the god of war, Ares, and aids the group in their journey (quest).

Time shall want her as his trophy.

Time refers to Kronos. He wants her to join his army, where he will treat her as his trophy. He calls her a diamond and states that if he can't have her, no one can.

(This part of the prophecy may or may not still be in play)

Destroy with a hero's final breath

Daedalus is willing to die for the Labyrinth's destruction. His life force is directly connected to the labyrinth, so when he dies, it 'destroys' the maze.

And lose a love to worse than death

Annabeth loses Luke when he gives his body to Kronos.
