MY HAND FAQING HURTS TO MUCH AGH
If you look carefully, the spikes' true color is grey, but once they penetrate through Lovi's body, they turn almost black(another clue to the meaning, also, jesus christ why did I say that like it was a normal sentence?!)
Also if you're not blind you can see the largest spike going in through the back of his head and out of the mouth, again, another clue(and the primary clue) to the meaning

Okay,
I have this idea that Romano is one of those characters that was hurt and abused(not physically) in his past OR like Russia or Germany, had a strange and/or violent past, this caused him to start his normal behaviors like cursing(alot)
The spikes going in represent the cause
The ends protruding are the effect, people touch them, they get hurt, this represent Lovi's cursing
Even if the spikes were gone, they would still leave 'scars'
And you should have guessed by now what the spike in the mouth means...

Tbh I haven't drawn proper gore in a while, so maybe this was my mind's excuse to do it
Maybe someday I should draw pastel-gore :3
