So this story sort of begins at the end of episode 18 of season 1 of The Vampire Diaries and then there's a little delay before episode 19. Here's a little summary of where we left off at the end of episode 18.

Elena returns home from the town's Founder's Day 150th Anniversary party. She goes to Jeremy's room to check on her younger brother.
He acts as though everything is alright and shrugs his sister off when she asks him questions. He's hiding his feelings toward her about the fact that he now knows the secrets she has been keeping from him about vampires in Mystic Falls.
Elena gets the hint that Jeremy doesn't really want to talk and retreats to her own bedroom. She is greeted by Stefan, who wants to apologize for his drunken behavior at the party. He tells her that he can't control himself and thinks it's in their best interest if he stays away from her. Elena tells Stefan that she isn't afraid of him and will continue to be there for him through his struggle.
Stefan returns to the Salvatore boarding house and finds Damon sitting down, drinking a glass of blood, while worrying about John Gilbert's words earlier that night. Damon notices how much Stefan is struggling and decides not to burden his brother with his problems. He leaves the room, setting the glass of blood on the table.
Stefan slowly walks toward the glass of blood, debating on whether to drink it or not. He gives in to temptation and downs the glass of blood.
