A/N This is long and more about a massive trauma. It is a little bit gory, has mentions about blood, wounds and death. So if this could be a trigger for anyone or just something you may not like, just be aware. Not really Schmico but please read!

The ER was in complete chaos as patients continuously came through the doors, there not being enough doctors or beds for the mass of them. There had been a critical accident not too far from the hospital and Grey-Sloan Memorial was where everyone was being sent, whether they had minor injuries or not. Levi was working on a patient who was covered in cuts and bruises but seemed to have no other substantial injuries when he was paged. Levi looked to see who it was, seeing Dr Grey was requesting him and apologised to the women he was with before taking off to the front doors. As he came over to them, he saw Dr Grey stood there with Link and Nico, holding medical bags.

"What's this?" Levi asked, looking to Nico with a confused look before he focused back on Meredith. She handed him a blue jacket that had the hospitals Logo and name on it, a serious look on her face.

"You're coming with us, this is a mass casualty accident and they need support on the scene," Meredith declared, watching as Levi put the jacket on and took another medical bag from a nurse standing by.

"Why me?" Levi asked as they walked out the doors, having to dodge paramedics and people running around. There were so many other doctors who she could've chosen to go with her, hell, even Casey would've been better because of his experience in the field. Levi wondered if he would be able to handle what was going on at the scene, he had never done anything like this before and he had seen the news and how manic everything was there.

"Because I know you'll do good," Meredith replied and they got in the back of a medical van. Levi watched as two others joined them, recognising them as doctors from the hospital but not really knowing them. Levi looked to Nico with an unsure look, wondering what he was thinking.

"You'll be fine," Nico reassured, his voice quiet so everyone else wouldn't listen in on them. "You know what you are doing, we will be able to help lots of people," Nico added, reaching over and giving his boyfriend's hand a gentle squeeze. He too was anticipating what they may encounter, knowing that it was going to be difficult and they weren't going to be able to stop for hours.

"Listen up," Meredith began, gaining the attention of everyone in the van. "It is going to be loud, there is going to be a lot of people hurt, you need to assess the situation. Perform a primary survey first on every casualty and until it's complete, do not be distracted by more superficial, non-life-threatening conditions," Meredith's voice was strong and confident, she looked at everyone, making sure they understood. "If they are screaming and shouting, they are breathing, go to the quiet casualties first," She added.

"What's the prioritising system?" Link asked, he looked ready to jump in a be a hero. Compared to Levi, he didn't look nervous at all but Levi's heart was thumping in his chest and he wanted to freak out but knew he had to keep a level head. Levi turned his head back to Meredith, waiting for the answer.

"We have to triage, green tag for non-emergent, yellow for delayed care and red tags need immediate treatment," Meredith replied, "Assess carefully, then tag and get the critical patients in the ambulances." Levi ran it all through his head even though he knew what to do, he didn't go through that crazy trauma training with Owen and April for nothing. "Schmitt, what else do we do?"

"Don't get in the way of first responders," Levi replied quickly and Meredith nodded. The van was slowing meaning they were nearly there, everyone grabbed hold of their bags tighter, preparing themselves for the worst.
