ZOOM BLIND DATE - GIRLS' FORM

Wala naman masyado. I filled this ip on a whim kasi nga moving on step. I don't know if considered bang seryoso pimagsususulat ko, but it is all true and correct.

Ang form na ito ay patterned sa first part ng Google forms ng Sobrang Late Show Blind Date - Zoom Call project last July 2020. So credits sa kanila for the idea. I edited and tweaked it around. Also, hanggang first page lang nakita ko. I didn't have the courage to actually fill up and participate that time. Lol!
