Noah found out about Caitlin's death a little over and hour after the cops did when Dan called him. He still blames himself for her death, and is struggling to live without her. He places fresh flowers on her grave everyday. Noah graduated Med school and became a heart surgeon. Victoria and Noah got married, and 3 years later had a little boy named Logan. Noah now drowns his sadness in his son, who makes him happy. He can't bare to lose someone else.
Dan visits Caitlin's grave everyday, but makes sure he goes to the grave after Noah has left to avoid confrontation. He blames himself for her death also, and feels guilty. He even begin cutting again. Losing her was too much of a pain to bear. He moved flats, because he couldn't stand looking at her empty flat everyday. He no longer lives with Phil, and is really struggling to move on.
