So, imagine if Touya was the reverse of what he is. Meaning, he has an Ice Quirk, but his body was meant to handle a fire quirk.
First, let me explain why Touya's quirk burns him, because a lot of people mix it up. His body is made to handle extreme cold, which means his average body temperature is higher than the average human's. The fire quirk brings that body temperature even higher, burning him. A normal person with a fire quirk would have a much lower temperature, allowing them to withstand the flames.
Back to the main topic, in this au, Touya has an Ice Quirk, but his body is designed for a fire quirk. So he has a low body temperature, and his quirk brings it lower, so he gets frostbite a lot more easily, and his skin freezes.
Touya's burn scars aren't burn scars here. They're patches of frozen solid skin turned a mottled blue-purple from the cold, and instead of staples, he has ice breaking out of his skin. Instead of burning himself, he's slowly freezing himself to death.
Your character meets Touya one day in an alleyway. He's using a lighter to try to thaw his skin out, but of course, he's fireproof.
