Great question, really great question.
The Doctor has a way of hiding everything about him, letting others know that "everything's all right" when really, it isn't. His companions that have been with him long enough can sense when he's lying though, but that's because they've been around him for a while, and got to know him very well.
Sherlock had deduced Mary (Morstan) Watson wrong, technically. In a way he deduced her correctly, but he didn't see everything about her. I'm not going to go into details about this because I don't want to give any spoilers.
Going on, Sherlock is brilliant but I'm not sure he would be able to know everything about him. He would definitely be able to know he'd been in the war, as he easily deduced about John Watson. I think Sherlock would also be able to tell that the Doctor had experienced many losses. The way the Doctor approaches things, looks at the companion (s) he has now, and/or anything else Sherlock analyzes.
I think Sherlock would see a war torn man, who has experienced many a heartbreak, and definitely intelligence. I think Sherlock would see the intellect in the Doctor and all of the smarts he possesses. If Sherlock spent enough time with him, he'd be able to see the dark side of the Doctor as well.
