Kenny had many problems. His parents were alcoholics, his dad beat him, his mom, and his siblings. He's addicted to various drugs as well as sex. He also doesn't really have any friends.
Some people would argue, after all, he has Stan, Kyle, and even Cartman. Which is correct to an extent, but none of them really care about him. Cartman is a racist, homophobic, lying piece of shit, and Stan and Kyle are too wrapped up in each other to pay attention to anything else. Everyone else thinks he's just trying to get laid when he talks to them.
Some people who get desperate might say, what about Tweek and Craig you hang with them sometimes. Craig is just someone to manipulate and occasionally borrow cigarettes or booze from. Tweek is a different story, he's the first person in Kenny's life who doesn't mind spending time with Kenny without reason.
Tweek makes him feel like he's not messed up. He makes him feel fluttery and sweetly sick. Although he knows there's no chance for them, Tweek is very much in love with Craig. He's not okay for Tweek anyway, but that doesn't mean he's giving up on Tweek.
Kenny is one selfish person, and he's not giving up without a fight.
