"Is Nayeon home?" I walked to the living room while Tzuyu was locking the front door.

"She probably went out with her friends. She always stays at living room whenever she is home. Let's go to my room." She leading me to her bedroom door and unlocked it. She opened the door and we went inside.

I roamed around her bedroom. A bookshelf of books, a wardrobe, a desk, a foldable mini desk beside the futon, a dog stuffed toy on the futon. I walked to the bookshelf, all were medical books and dictionaries, some of the books were Chinese. I then walked to the futon sat down brushing the fur of the cute plush toy.

"Her name is Minjoong" Tzuyu standing behind me. She then sat down beside me hugging, stroking the puppy plush toy. Her action was so cute like a child. "I miss Gucci." She murmured.
Heh? Brand?

She then took out her phone showing me her wallpaper. It was a photo of her and her dog. "This is my dog, Gucci."She sniggered. I laughed at the extraordinary name.

"I also had a dog, Ray. I missed him. He died of old age." I looked down as I felt the wet hot tears fill up my eyes. I blinked my eyes. A single drop of tear welled up from the corner of my eyes. She lifted up my chin and wiped the tear threatening to drop from my chin. Then, a warm hug embracing my torso following by patting on my back. I leaned my chin against her shoulder and pleasingly placed my hand around her waist. "Thank you, Tzuyu."

"Let's start our lesson before it is getting late." I patted on her back and left each other embrace.

"Earlier in the morning, there was a case study discussed in grand rounds. It's about a 25-year-old woman presents to the emergency department with a chief complaint of headache, back pain, and fever.

A week and a half ago, she had some vaginal itching and discharge, which she thought was a yeast infection so she used Miconazole. But after 5 days with no improvement, she decided to go to an urgent care center for evaluation. At urgent care, she was diagnosed with a urinary tract infection and a possible sexually transmitted disease, for which she was given a number of antibiotics, including an injection and a prescription for Cephalexin.

She states that she is still no better despite taking all the prescribed medication from the urgent care clinic. She has ongoing vaginal discomfort, mild discharge, and dysuria, and in the past 2 days she has developed diffuse back pain, a bad headache that is worse on standing or moving, and subjective fevers. She denies photophobia, neck stiffness, nausea, vomiting, or other complaints. She says that she had unprotected sex a few weeks before this all began.

Her chart reviewing, she is afebrile with normal vital signs at triage. Her temperature is 37.3 °C, and the doctor happened to see her walking into the exam room and noted that she was moving slowly for a woman her age. On bedside exam, she has no photophobia and a supple neck. Her jolt sign is positive. She has mild bilateral CVA tenderness that is worse on the right. Her abdomen is benign, and her neurologic exam is nonfocal. Urinalysis shows 12 white cells/hpf and no bacteria. Complete blood count and metabolic panel are both normal.

There are three possibilities based on her diagnosis. It could be pyelonephritis, genital herpes, and herpes encephalitis. So urine culture, lumbar puncture with cerebral spinal fluid, and herpes simplex virus polymerase chain reaction were ordered. The result is genital herpes. The lumbar puncture results were as follows: glucose normal at 80, protein elevated at 250, white blood cell count elevated at 315 with a lymphocytic predominance. The herpes simplex virus polymerase chain reaction is positive. She was admitted.

She has the symptoms of genital herpes like the infected site start to itch, or tingle, and have headaches, body aches, and fever. However, the clinical test and urinalysis results didn't match until lumbar puncture and herpes simplex virus polymerase chain reaction were ordered. Why is that?" Tzuyu tilted her head with a perplexed look.

"The clinical presentation and urinalysis results could certainly be due to partially treated pyelonephritis, although bilateral pyelonephritis would be unlikely. So the doctor looks for another explanation for the vaginal symptoms and the headache with a positive jolt sign. She presumably has been treated for yeast, gonorrhea, and chlamydia, so the ongoing vaginal symptoms might be a mild case of genital herpes. She could also have meningitis due to herpes. The decision was made to proceed with a lumbar puncture and a herpes simplex virus polymerase chain." I explained. Realization bringing to her face.

Tzuyu quickly stood up, placed the fordable mini desk in front of me and took out a book from the bookshelf. She sat beside me, turning the page. She stopped and put the book on the desk.

I was impressed by her note-taking skills. Sticky notes were all over the book. All were translations, interpretation, and drawings. "Sorry. It's a bit messy." She felt self-conscious when I looked at the book wide-eyed. "No. It's splendid. Tzuyu, you are very hardworking." She smiled embarrassingly.

"I don't understand this."She pointed at a case study.

I explicated all her questions. What surprised me was most of the question related to allergy and immunology, which was my specialty. "Tzuyu, do you have interest in clinical immunology?" Tzuyu nodded heartily. I flashed a beam of satisfaction to her. "That is my specialty."

The corners of her mouth turned up and there was a sparkle in her eyes. "Myoui Mina, are you a goddess?" she blurted out. I tittered and jokingly said, "No, I'm a princess."

"Yes, my princess."Tzuyu jestingly took my left hand and lifted up to her lips. Chu. She tilted her head upward and looked at me in a flirtatious manner. I could feel the blood rushing to my cheeks. "Why is your face burning like mad?"She teased. I covered my cheeks in embarrassment. "Adorable." She pulled down my hands then poked my cheeks. "Kawaii~" I thought I would die from red-faced under her strong gaze.

"You have many beauty marks on your face." She touched the mole on my nose.
"Are you a dermatologist?" I scoffed, trying to change the scorching atmosphere.
"No. But I want to check them and kiss them." She leaned forward dangerously close to my face.
My face once again coloured with red.
"You are so fun to tease with." She moved away from my face smiling triumphantly.
I coughed nervously and looked at my watch subconsciously. Time to go home.

"Good night, Tzuyu." I stood up.
"Sleepy? My princess, wanna me carry you in bridal style to bed?" She stood up and stepped closer to me. I lower my head, didn't dare to look into her eyes.
"I mean goodbye, Tzuyu. Not goodnight. But goodnight too." My stuttering was met with a guffaw from her.

"Thank you for being my tutor. I learned a lot today." I nodded in approval, still lowering my head. She surprised me by her hug, she didn't seem like a person who likes skinship. Not that I'm complaining, I like the hug though. Her height is perfect for me to lean my head against her shoulder.

We walked out the door and Nayeon was there sitting on the couch on her phone.
"Nayeon 언니." She seem shocked when she saw it was me.
"Hi. Princess Mina." She let out a nervous chuckle.
"Nayeon 언니, I never saw you like that before." Tzuyu mocked.
Nayeon glimpsed her then facing me "Princess Mina, why are you here?"
"Tutoring Tzuyu." She titled her head questionably. "You're a doctor?"
"Yup."
"Daebak!" Nayeon eye-wided, thumbed up. Her expression was so funny. "It means awesome in Korean." Nayeon explained without me asking.
"I got to go, Nayeon 언니. Goodbye."
"Tzuyu, see you tomorrow."


"Princess Mina, the signal moved this morning. Now is at Minato ward."
"Good job, Genos. Keep follow." I hang up the phone.

I entered the entrance. Father and mother were sitting on the couch, watching TV. Father saw me. "Mina, come here." I sat down beside father. "Do you know Prince Yuta Nakamoto?"

Nakamoto? Why suddenly mentioning that family member? "Yes, father."

"Well, I met him today, he helped us in persuading MT Corporation which the contract finally reached." Father sighed.
I had a bad feeling. Nakamoto was the business aristocratic family. Almost half of Japan's economy is under their manipulation. Although we were the two most well known aristocratic family in Japan, but we didn't cross each other path because our viewpoint and field didn't match. They were the typical businessman, ambitious and cunning. He will not helps father if there was no benefit involved.
"But he requests for having dinner with you."
Why he suddenly had interested in me? Tomorrow will find out his intention.
"No problem, father. I will go." Father sighed deeply and patted my arm.
"Alright. Goodnight." They stood up. "Goodnight." I sat there in contemplation.
