Current mental state
The patient is calm, alert and responsive. His concentration is not good. He does experience hallucinations every night. Delusional ideas and thoughts noted. Sleep and appetite aren't fair, since the patient is suffering from insomnia, nightmares and sleepwalking. Attitude changes from time to time. Unable to recall past or recent events. No Suicidal ideation noted. Anxiety attacks increased.
Current Physical state
Weight loss.
Medications
Venlafaxine, 37.5 mg orally twice a day
Sertraline, 200mg orally once a day.
Psychotherapy
Talk therapy.
Cognitive behavioral therapy.
Eye movement desensitization and Reprocessing (EMDR).
