I walked into Emily's room, where she was sitting on the edge of her bed, looking a bit uneasy. The room had that classic hospital smell, and the soft hum of the ventilator was the only sound. Emily looked up as I came in.

"Hey, Emily. How's it going today?" I asked with a friendly smile.

She sighed and said, "Not great. My chest is still feeling tight, and it's hard to catch my breath."

I pulled up a chair next to her. "Got it. Let's go over what's been happening. When did the tightness start? Was it sudden, or did it build up over time?"

Emily thought for a moment. "It started a few days ago. Last night was the worst. My inhaler didn't seem to help as much."

I nodded and made a quick note. "Have you been using your inhaler like we talked about?"

Emily shook her head. "I've been using it as I'm supposed to, but I think it might be the dust from the construction next door. It's hard to avoid."

"Dust can definitely be a problem," I said. "We'll look into adjusting your medication to make sure it's working for you. In the meantime, try to steer clear of the dust as much as you can."

Emily's eyes were still worried. "Is there anything else I can do to feel better? I hate feeling like this."

I smiled reassuringly. "We'll work on getting your treatment just right. Meanwhile, avoiding those triggers is key. We'll also go over some other tips to help with your breathing."

I pulled out my stethoscope and checked her breathing. "Let me listen to your lungs and see how they're doing today."

As I listened, I heard the familiar wheezing. "I'm hearing some wheezing, which fits with what you're describing. We'll need to tweak your meds and keep an eye on this."

Emily seemed a bit more hopeful. "Thanks."

I made my way back to the nurse's station, noting that it was already 6 PM and I'd been here since last night.

The day had been a whirlwind, and I was just trying to catch up. Lena was at the station, sorting through some papers.

"Hey, Dr. Knowles," Lena said, glancing up, "We've got a few updates for you. There's a new case in Room 4—a young woman with suspected appendicitis. We're waiting on a few test results, but it looks like she might need surgery soon. Also, don't forget to check on the charts Dr. Perez mentioned."

I nodded, looking over the notes. "Got it. I'll head to Room 4 first. How's everything else going?"

Lena sighed, "It's busy but manageable. If you need any help or have any questions, just let me know."

"Thanks, Lena," I said, slipping back into my professional groove. "I'll take care of it."

I walked down the hallway to Room 4, my mind focused on the young woman who needed my attention. As I approached the door, I knocked gently before entering.
