Cara panics and looks at Raf, "what the fuck do I do?" she says.

"Don't panic... We're not far from Holby we can go there" Raf says in a rushes tone.

"But what if this baby decides it's coming now Raf?" Cara panics but screams as she feels a pain down her back.

"It won't... You've probably got ages yet" Raf says.

"I was 3cm dilated this morning before we left Raf...I didn't want to panic you" she says.

Cara looks at Raf who is silent, "you should of told me, oh Cara" Raf sighs.

Cara looks away from him, "Cara this is more important " he says as she feels the contractions getting stronger.

"What?" she says as she breathes unevenly.

"This baby is more important that the meal... I should of noticed there was something like this up" Raf says.

"Don't blame yourself, please" she groans. She cries in pain again as she grips the chair. "Raf please, I don't know what to do and this baby is coming" she screams.

Cara breathes unevenly as Raf drives. "Raf we're gonna have to stop" she says as the pain becomes unbearable.

"It's fine were nearly there, twenty minutes tops" Raf says.

"We don't have twenty minutes!" Cara panics.

"Raf this baby is coming now I swear" Cara panics as she holds her bump. "Raf for fuck sake pull over" Cara cries.

Raf says no more and pulls over. "What do you want me to do?" Raf panics.

"I don't know Raf but this isn't good" she cries.

"Breathe!" Raf says as he strokes her knee.

"I feel the urge to push but I don't know what to do" she panics.

Cara tries to stop herself from pushing but she can't help it. "Right well I guess I'll have a look?" he suggests as he goes to take her jeans off but she doesn't let him.

"No" she says as she pushes his hands away.

"Right out you get" Raf says as he gets out himself.

"Out I get? Where the hell are we going?" she says as they are in the middle of no where.

"In the back" Raf says as he opens the back door.

"Now isn't the time for fucking games Raf!" Cara shouts.

"Cara if you get in the back I can take a look and no one will see, it'll be easier, there's more room" Raf says in a calm tone.

Cara starts to cry and Raf helps her out and into the back. "There we go, nice and easy" he says as he helps her lie down.

He grabs a blanket out of the bot and rolls it up and gives it to Cara to use as a pillow.

"Can I take your jeans off?" he asks in a calm tone.

She shakes her head and cries out as another contraction happens. "Well I'm afraid I'll have to if you really think the baby is coming" he says.

She finally agrees to him taking her jeans off. "No I don't want you examining me, just get me to the fucking hospital" she cries.

Raf laughs a little "come on Cara I am a doctor, I can do this now and then we can go to hospital" Raf says.

"Fine" she huffs as he puts another blanket around her legs for some privacy.

He takes her jeans and pants off and checks her over. Raf looks up at her and reaches for his phone, "what's wrong Raf?" she panics.

Raf says nothing and scrolls through his contracts, "Raf say something" Cara shouts.

"I'm ringing Serena" he says.

"Why?" she panics.

"Just guidance" Raf explains, Cara can't help herself but push.

"Hello Serena?" Raf says as Serena picks up the phone.

"Yes Raf what's wrong? Aren't you meant to be on holiday?" she asks,

"Well we was but Cara's waters have broken" Raf says. "And...we're in the car" he adds.

"Oh right are you coming to Holby?" Serena asks.

"Erm no...because it looks to me baby is coming now...right now" Raf says which makes Cara panic. "I think I can see some of the head... I'm not sure" Raf says as he looks at Cara again.

"I'll send an ambulance on it's way" she says.

"Wait what do I do? I've never delivered a baby before!" he says.

"Oh fucking brilliant, I want the organ grinder but end up with the fucking monkey" Cara pants as she makes her way through another contraction.

Raf can't help but laugh. "Right how far out is the head?" Serena asks.

"I can see the hair if that means anything" he says and Cara looks at him not best impressed.

"Right Cara needs to give little pushes to help the baby's head out" Serena explains.

"Serena says little pushes to get the baby's head out if that's possible?" he asks her.

"Fucking bitch she is... I can't!" She screams as she just pushes.

"Give it a go" he says as he sets his phone down and puts Serena on speaker.

"Ahh that's it Cara, just like that, there we go" Raf smiles as the head comes out.

She screams as it's rather painful, "your doing really well babe" he smiles.

"Don't you ever come near me ever again!" Cara screams.

"Okay that's fine" Raf says as he knows she's not being serious.

"Okay now support the head and make sure that baby turns so it's face is up" Serena says to them and Raf does what she says.

"Baby's head is out Caz" he says as he supports the head and Cara continues to push.

Cara looks at Raf as he rubs her knee with his free hand, "your doing so well princess keep going" Raf smiles.

"Chin into chest and push" Raf smiles.

"When the baby is fully delivered I need you to place the baby onto Cara's chest and finds something to wrap the baby up in" Serena says.

"Will do" Raf says.

"This is all so real now" Cara cries as she pushes again.

"That's it nearly there, your doing ever so well" Raf smiles.

"Looks like baby's excited to see it's mummy, fast mover" Raf smiles.

"Shut up Raf" Cara screams.

Finally the baby is delivered and Raf places the baby up onto Cara's chest.
