A little girl

After a painfully silent drive we arrived at a hospital just outside Buenos Aires. He reached for my hand so I held it tight.
"It'll be ok.It can't happen again" I said
"I can't loose her.Not after everything"
I could see it wasn't the right time to ask so I decided to ask later.We navigated down endless corridors until Germán stopped.
"What is it? Germán you're scaring me"
I looked through the door and saw a girl around my age lying still on the bed with a little girl about 10 crying beside her.
"Is it..."
"Yes... I had no idea"
We went in and the little girl glanced up at us.
"Are you..."
Germán slowly walked over to the bed before hanging his head down in shame.
"I never tried to help. I never searched or anything.How could I?"
"It's not your fault! You couldn't have known"
"I should have stopped them" and with that he fell silent.
Who was 'them'?Why didn't he know about this?
