In Gen 4, if the player blacks out or uses Teleport when the Cinnabar Island Pokémon Center was the last Poké Center they visited, they will be taken to the last Poké Center they had visited prior to Cinnabar Island instead. This is likely to prevent the player from being permanently stranded on Cinnabar Island if their only Pokémon can't learn Surf or Fly and they have no means of obtaining another Pokémon.
