1 month later

"Hey, how was work?" Jacob asks as soon as I walk through the door.

"Fine." I say shortly. I walk directly to our bathroom and peel of my scrubs, washing off today's work in the ER.

After my shower I go to the kitchen and look for food.

I hear the twins behind me. They both grab my legs and hang on, not letting me move without dragging them with me.

I sigh in defeat. Combined with a long work day I don't have the energy for this.

"God, Evi." I hear Jacob say under his breath.

"What? I'm tired and not up for this tonight Jacob."

"You know what? It seems like you're always tired! You're always not up for it! God Evi you don't even take the time out of you're so called busy schedule and tiring work day to be with our children!"

They both started to cry at his sudden outburst.

"Can't you give me a moment of peace! I'm so sick of you on my case all of the time!"

"On your case? On your case!?"

"Yes on my case! would you quit nagging me about every little detail!"

"You don't do anything! You go to work, you come home, shower, and go to bed! I haven't touched you in weeks! You haven't paid any attention to the girls! It's like I'm married to a freaking wall!"

"My son died! What do you  expect me to do!" By this time tears we streaming down my face and the girls were screaming, crying too.

"Dammit Evi! My son died too!" Tears now wet his cheeks also.

I have no clue what to do so I grab my keys and run out the door.

"Where are you going?" He asks, but I don't answer.

I get into the car having no idea where I'm going with tears streaming down my face.

It's so dark out I can hardly see, and with the combination of my tears I really can't see at all.

How could he say those things?

I can't believe he did.

I feel myself drift the the left.

Before I can fix it and move back to my lane I see two bright headlights.

My car hits the left side of a truck and flips a couple of times.

My head hits the windshield so hard that the whole thing shatters.

My chest is aching.

My head is bleeding, all I can see is blood.

My leg is snapped at the knee, I can feel it.

I hear the faint sound of an ambulance before I'm out like a light.

Jacobs POV

"Shh, shh it's okay. It's okay." I say rocking the girls, trying to get them to calm back down.

Where is she?

Why isn't she back yet?

I hear my phone ring in the other room.

Hopefully it's her.

"Hello is this Dr. Jacob West?"
"Yes? Who is this?"
"We regret to inform you that your wife Dr. Evangelina Stevens-West was just admitted to the emergency room."
"Oh my god! What happened?!"
"She was in a car accident-"
"Okay I'm on my way."

I quickly grab the twins and put them in the car.

Once I arrive at the hospital I drop them off at the 24 hour day care and head to the ER.

I see her in trauma room 1 and immediately go inside.

"Oh my god Evi!" She was wide awake.

Below her knee there was hardly any skin  I could see that the bone was cleanly broken in two.

There were cuts all over her, and a huge gash going across almost her entire forehead.

The plastic surgeon, Dr. Green was picking glass out of her arm, and the orthopedic surgeon, Dr. Marin was cleaning her leg.

"Hey! My husbands here. Isn't he handsome! Hahahaha."

She was clearly on medication.

"Hey, are you alright?"

"I'm great! How are you?" She slurred.

"I'm fine. Where's her chart? And what the hell drug is she on?"

After looking through her chart I notice she hasn't even had a full body work up or a head CT or any sort of X Rays.

"Okay this is my case now. You all can leave. I'll page you to continue fixing her cuts and leg as soon as she has a CT and a work up."

They obediently leave even though I'm not supposed to have any family related cases.

"Jacob! Why did you kick my friends out?"

"Hold still. I need to see if you have any other injuries."

I check her arms and other leg. Nothing. Her stomach has no internal bleeding, that's good.

I'm almost finished filling in her chart when I see a growing spot of blood on the top of her shirt.

I pull her shirt back and am horrified by what I find.

"Page Montgomery, and Rios! And get an OR available! Stat!"

I push her though the door and to take a quick head CT, just to be sure.

When I read the scans there's nothing, thank god.

"Jacob. Why on earth are you yelling so loud?" She asks me.

With tears brimming my eyes I tell her that she has to go into surgery.

"Dr. West, I got your page." Dr. Montgomery says.

"It's Evi. She got into a car wreck. I need to preform surgery immediately."

"You know the rules. You cannot be her doctor-"

"She has a shard of glass in her heart."

Her eyes go wide.

"Nobody can preform this surgery but me and you know that."

She sighs in defeat. "Let's go. I'm scrubbing in with you."

Once Evi is prepped and on the table, about to go under the anesthesia I walk to her side.

"Jacob? What's happening?"

"You're about to have surgery."

"I'm scarred."

"Don't be."

And she is out.

"Are you ready West?"

"Yeah, scalpel."

I start by lengthening the incision where the glass enters her chest.

"Okay, ready with the clamp?"

"Ready."

I slowly pull the shard of glass from her heart and I hear the monitor flatline.

"Dammit! Push one of epi!"

I suture her heart and grab the paddles.

I feel tears brimming my eyes.

"Charge to ten."

Nothing.

Tears are streaming down my face. 

"Charge to twenty."

Nothing.

"Push another of epi." I choke out.

"Dr. West, she's gone."

"No! No she's not. Charge to twenty."

And finally, the monitor starts beeping again.
