When Deceit/Janus is fighting Lilypadton and he (Pat) hits Thomas, ONLY ROMAN reacts to what happened.

It isn't until it's directly pointed out to him that Patton realizes what he's done and looks like he regrets it

This gives me two thoughts:
Patton didn't even realize Thomas got hurt. He was so wrapped up in fighting Janus that he completely forgot WHO they were fighting for, and I don't think he even realized that he hurt Thomas at all. I think that shows how stuck in his own head he was; getting too deep into the morals and rights and wrongs and questions that he lost sight of the PERSON who started all this.
Roman says later in the video that he's Thomas' hero, that he basically thinks of himself as Thomas' knight in shining armor, and we've seen before that if he thinks a side is bad for Thomas, he will go for their throat at any opportunity. I think, subconsciously maybe, he blames Janus for jumping out of the way. He might also be mad at Patton, idk.
Roman didn't forget who he was fighting for, so he reacted immediately.
Janus already knew this was happening, so he didn't even falter on it.
Patton had completely forgot about Thomas, so he had to be told directly.

Logan: Hey Virgil?
Virgil: Yeah?
Logan: I got called a VISCO girl...
Virgil: Oh shit, I'm so sorry Logan. Are you oka-
Logan: Do you want to wear matching scrunchies? I got you a purple plaid one, I'll still have my blue plaid one though. Just to stay on brand, you know?
Virgil:
Virgil: Abso-fucking-lutely, hand it over.

Deceit: Excuse me, what?
Virgil: I hate both of you.
Logan: *deep sigh*
Roman, who sadly already knows what that means: Deceit, do you even know what that means?
Deceit: Obviously not.
Patton, quietly: Your chest, Deceit.
Deceit, now hitting Remus with a flip flop: You perverted idiot! You'd have me show them my body as a distraction!?
Remus: IT WAS WORTH IT!
