Kokichi didn't know when he contracted the disease, but he soon found himself diagnosed with a sickness that could be found in fairy tales in which it is caught through a one-sided love. Though if you squint at it a bit, it was unrequited love. It started off with light coughs from Kokichi and his throat hurting a little bit, but Kokichi could just pass it off as mucus being stuck in his throat.

He never looked too into it, when he suddenly started erupting violent coughs that made him start coughing up bits of blood. Kokichi wasn't the type to attract any negative attention towards him, because he didn't want anyone's pity since it made him feel weak. Even if his ultimate supreme leader title was scratched off when they woke up from simulation, he still considers himself as a supreme leader.

If Kaito can claim himself as the Ultimate Astronaut during the games despite never going up into space. It was fair enough for Kokichi to hold onto the false hope of him ruling over his small prank group called D.I.C.E. But it was no longer the games in which they were trapped on school grounds for who knows how long, they now had to face the real world, something that they all have to get used to.

Shuichi soon started dating Kaede and years later, he announced their proposal. Sure Kokichi felt his heart break slightly, but he was happy that Shuichi was able to find a girl who he loved. Though that didn't stop the heartburn that suddenly started building up to excruciating pain weeks after the announcement.

Though Kokichi was able to keep it to himself,he always has been. Of course it would soon be slowly leaking into his life little by little. Having worried glances be sent towards his way when he started coughing violently all of a sudden during Thanksgiving. Kaito noticed how there was a bit of blood coming out of Kokichi's mouth and he frowned, because he also had an illness that involved him coughing out blood.

So after the dinner, Kaito confronted Kokichi, pulling the other purple-haired male into a room."Kokichi when the hell did you get the illness?" asked Kaito and Kokichi rolled his eyes."C'mon Kaito, don't be some dumbass and think that I have the same case as you, because I don't." said Kokichi. Kaito sighed before shaking his head, no wonder he gave up on trying to change the boy to be a better person and possibly open up.

"Well we're going to the hospital to check on it." said Kaito."I didn't know that the idiot can drive, how many accidents did you get into?" asked Kokichi, giggling to himself and Kaito arched his eyebrows."Just go to the hospital tomorrow and see what's wrong. Go call and uber if you want." said Kaito, patting Kokichi on the shoulder before leaving the room.

Of course, Kokichi won't listen to Kaito and just shut himself into his house, since he already knew what disease he was diagnosed with, he lied to Kaito when the ex-astronaut asked if Kokichi ever went to the doctor. Kokichi brushed it off as heavy allergies, and well Kokichi was taking advantage of Kaiot's dumbassery since the taller male doesn't know that you can't cough up blood from allergies.
