Overall thoughts:
IN CASE YOU IGNORED THE WARNING IN THE BEGINNING ABOUT SPOILERS AND YOU HAVEN'T SEEN THE EPISODE, THIS PARAGRAPH GIVES AWAY THE ENTIRE ENDING.
I liked how they didn't make Sherlock this extreme badass character who does everything right. Especially when him and the cabbie are talking at the table, and he's holding the pill, Steven Moffat didn't make Sherlock eat the pill and have him magically have the good one. He made this extreme plot twist that all tied together. Not only that, but you can tell that when Sherlock was holding the pill, he seemed unconfident and vulnerable, as if he didn't know what would happen, so we know that Sherlock does have a vulnerable and weak side to himself.
There is also the fact that it would have been expected that at the last possible second, Dr. Watson would magically burst through the doors and save Sherlock. But no. Moffat cleverly made it that Watson ended up in the wrong room and saw the two talking, but they couldn't see/hear him shouting for Sherlock. Then, (not at the last possible moment, but somewhere in between dialogue), Watson shoots the cabbie. I think that perfectly displayed how he didn't knwo what was going on and just guessed what was happening. For all he knew they could have been old colleagues and the cabbie wasn't the murderer at all. Also, I liked how he shot the cabbie in the middle of his sentence, because shooting him ight after would suggest that Watson knew what was going on, which he didn't.
In just an hour and a half, they were able to display Sherlock and Watsons thoughts, but also give the viewers new looks on the serial killer and Mycroft, instead of just "They were villians."
I love this show so much.
