At the beginning of your relationship, you'd be shy and awkward about this topic. You'd keep it to yourself and wouldn't ever let on about it. You wouldn't want to make him awkward or weirded out, so you'd keep your symptoms to yourself as best you could, and just go about your day-to-day life.
He would likely be able to tell when you were on or near your period, though, just because of how observant he is. He'd notice small changes in your moods and, depending on how your body physically reacted, he would notice physical changes, too (like tender breasts, bloating, headaches, etc.). He'd never outright ask when you were on your period - he'd know better - but he would ask how you were feeling and would ask if there was anything you could get for you.
After being together for a while, you'd start to open up more and be more comfortable expressing your discomfort when it was that time of the month. Spock would not be uncomfortable by the topic; it was a normal, biological process and he understood that logically, he should be supportive and treat it like any other bodily function. He would, however, sometimes word things a little weirdly or bluntly: "Do you have sufficient coverage for your flow today?" "Are your uterine walls contracting again?"
He'd mean well, though, and would want to help you feel better any way he could. He'd probably go to Christine for medical advice if you had very bad cramping or other symptoms like an upset stomach or bad headache. He'd fetch medicine for you and would make mental notes of Christine's suggestions.
If you ever went on an away mission whilst on your period, Spock would ensure to discretely bring extra clothing and hygiene products just in case.
He'd never coddle you, nor would he treat you like an invalid. He'd know you were well and capable of taking care of yourself, but he would want to help if he could. He'd offer cuddles and massages when in private and he'd check in during the day if he could. You'd truly appreciate everything Spock did, and would love him even more for how he treated you during this time.
