Dean, Cas, and Sam are on a hunt across the country, in Florida. They all hate going that far from home, but hunters from down there called and they needed help quickly. Everyone packed up their stuff as fast as they could, not double checking to make sure if they had everything, and rushed to the case.
When they were about six hours away from home, it dawned on him... Dean forgot his anxiety and depression meds. The meds that practically keep him functioning. He hopes to god, even though he isn't listening, that it doesn't effect him too much.
Dean's let down, as always, on the second night of the case. The boys were all going to go to dinner, but Dean felt drained. He just wanted to lay in bed and be in silence since he hasn't had that in two days.
Sam and Cas didn't think anything of it, Dean likes his alone time just as much as anyone else, so they went to dinner without him, leaving him alone in the motel. When they left, Dean sighed, laying down in his bed with greasy hair, unwashed clothes, and a terrible odor radiating off of him. He had no desire to fix it, so he went to sleep.
The next morning, Dean felt fine, great, actually. He was up at 8, taking a shower, doing all of his normal morning stuff, so he assumed that the night prior wasn't because he's off his meds, that he just needed some quiet.
This stayed the same for a total of four hours, after that, Dean kept getting irritated at the smallest things, especially when someone touched him. Sam noticed this and when Dean stormed out of the bar they were at with the other hunters that night, he knew what was wrong.
Sam followed Dean out and saw him sitting on the ground outside of the bar, taking deep breaths, trying to hold back tears. "Dean," Sam said softly, sitting on the ground next to his brother, making sure not to touch him. "What's going on?" Sam asked, his voice still soft so he doesn't sound mad.
"I- there were too many people, noises. It freaked me out." Dean admits, breathing rapidly as he thinks about it. There were people everywhere, touching him as they walk by, shoulders brushing as they order drinks, everyone talking and some people cheering for some football game that was on. It was way too crowded in there for him.
"Dean have you been taking your meds?" Sam asked, worried about his brother. When he goes off his meds, it gets bad quick.
"I- I need to go. I can't- I can't stay here. I can't-" Dean stuttered, breathing picking up quickly.
"Okay Dean, you can go, it's okay. We can find a ride. Just..." Sam said, sighing. He could tell that Dean's not taking his meds, and he didn't want Dean hurting himself. "Let me have your knife." Sam says, holding his hand out.
Dean nodded and grabbed the demon-killing blade out of his boot. He didn't care what it took, he needed to be out of there. He also emptied out his waistband of anything that could hurt him. He knows the drill. "Thank you. Please be safe on the way back. Text me as soon as you get there, okay?" Sam said, worried about Dean. He hates the fact that he has to take care of Dean like he's the older brother sometimes, but it's necessary.
Dean nodded, rushing off to his car, starting it, and driving away quickly. Sam sighed as he equipped himself with every weapon Dean laid out.
Dean rushed to get back to the motel. He needed to be in silence for a while before he could even think about talking to anyone. He got there, sent a quick message to Sam saying, 'back.' And laid down on his bed, breaking down in tears.
